锁骨钩钢板治疗肩锁关节脱位和锁骨远端骨折45例

Treatment of acromioclavicular dislocation and distal clavicular fracture with clavicular hook plate of 45 cases

目的:探讨肩锁关节脱位和锁骨外端骨折的治疗方法和疗效。方法:对45例肩锁关节脱位和锁骨外端骨折切开复位锁骨钩钢板固定,术后早期功能锻炼。结果:术后45例患者X线片检查肩锁关节间隙正常;或骨折对位好,锁骨钩钢板位置好。术后平均随访15个月,疗效评价:优39例,良5例,差1例,优良率为97.8%,均无钢板断裂、感染,关节功能恢复良好。结论:锁骨钩钢板适合局部解剖特点,方法简单、稳定可靠,有利于患者早期功能锻炼,关节功能恢复好,是治疗锁骨远端NeerⅡ型骨折和肩锁关节TossyⅢ脱位的良好方法,值得临床推广应用。

Objective：To investigate the treatment method and effect of acromioclavicular dislocation and distal clavicular fracture.Methods：45 cases acromioclavicular dislocation and distal clavicular fracture patients were treated with clavicular hook plates.They received post-opration rehabilitation training early.Results：45 patients＇ articulatio acromioclavicularis through X-ray sign were well after surgical operation,or the fracture contraposition were well,the position of clavicular hook plate was well.All the cases had been followed up for an average of 15 months.Satisfactory results were obtained,excellent in 39 cases,good in 5 cases and fair in 1 case.Superier rate was 97.8%.The breakage of the clavicular hook plate and infection were not observed.The activity of shoulder joint was normal.Conclusion：The clavicular hook plate is a good choice for Neer Ⅱ distal clavicular fracture and Tossy Ⅲ acromioclavicular dislocation because it is easy to handle,adapting to the trait of regional anatomy,and provides stable fixation and allows early mobilization.The way is to be worth generalization at clinical.
