二维超声心动图监控下心包腔内介入治疗恶性心包积液(附22例分析)

Pericardial Interventions for the Treatment of Malignant Pericardial Effusion under 2-Dimensional Echocardiographic Guidance (Analysis of 22 Cases)

目的 探讨二维超声心动图监控下心包腔内介入治疗在恶性心包积液治疗中的价值。方法 在二维超声心动图监控下对２２ 例恶性心包积液患者行心包穿刺抽吸，并注入卡铂行心包腔内化疗，部分患者心包腔内合用γ－ 干扰素行免疫治疗，同时行心包积液细胞学检查。结果 治疗有效率达７２－７ ％ ，无一例出现严重并发症，细胞学检查阳性率为８２ ％ 。结论 在二维超声心动图引导下行心包穿刺抽吸并行心包腔内治疗，对治疗恶性心包积液有良好的治疗效果，同时心包积液细胞学检查在心包积液诊断及治疗中有一定的价值。

Objective To investigate the effect of pericardial interventions for treating malignant pericardial effusion under 2-dimensional echocardiographic (2-DE) guidance. Methods 22 cases were studied. Under 2-DE guidance, pericardial puncture was completed and percutaneous and pericardial effusion was drawn, then chemotherapy with carboplatin in pericardium was taken (γ-interferon was added to partial patients). Meanwhile, cytological test of pericardial effusion was done. Results In all the 22 cases, the effective rate was 72.7%, no serious complication was found, and the positive rate of cytology was 82%. Conclusion Pericardial intervention was effective for treating malignant pericardial effusion under 2-DE guidance and cytological test of effusion is valuable in the diagnosis and treatment. [
