摘要
目的 实验性研究急性坏死性胰腺炎早期肾损伤的发病机制及生长激素和生长抑素的治疗作用。方法 经胰胆管逆行注射3 .5% 牛磺胆酸钠2.5ml/kg 建立大鼠ANP模型,测定血淀粉酶、内毒素、IL- 1、IL-6 、IL- 8 和TNF-α。观察肾病理变化和细胞超微结构改变,同时用RT- PCR测定TNF- αmRNA表达的变化,探讨生长激素和生长抑素的疗效。结果 ANP早期体内细胞因子和炎性介质过度升高,与肾损伤程度有关。生长激素和生长抑素可以降低肾TNF- αmRNA表达,减轻炎性介质的释放,以及减轻ANP的肾组织的炎性反应。结论 生长激素和生长抑素通过降低ANP炎性介质的过度释放,减轻SIRS,抑制肾组织的炎性反应,对ANP时肾损伤有重要保护作用。
Objective To investigate the mechanism of renal injury in early acute necrotizing pancreatitis models and explore the effect of somatostatin and growth hormone on rat ANP.Methods Retrograde pancreatic injection of 3.5% sodium taurocholate 2.5ml/kg in rats was used to establish ANP models.Levels of serum endotoxin、amylase、IL-1、IL-6、IL-8 and TNF-α were measured.Microscopic and electronmicroscopic examinations of the kidney were observed.Renal TNF-α mRNA was determined by reverse transcriptive polymerase chain reaction(RT-PCR).Results Endotoxin and inflammatory mediators were remarkably elevated,correlated with the damage of kidney.Somatostatin and growth hormone inhibited these inflammatory mediators and TNF-α mRNA over-expression and reduced the intensity of the kidney damage.Conclusion Somatostatin in combination with growth hormone can inhibit serum inflammatory mediators and renal inflammation,which prevents the development of ANP and protect the liver and kidney from damage.
