直肠癌术后吻合口漏的防治体会

Clinical research of prevention and cure of postoperation stomal leak of rectal cancer

目的探讨直肠癌保肛手术后吻合口漏的防治方法。方法回顾性分析2005年6月至2010年6月中国中医科学院广安门医院肛肠科直肠癌保肛手术137例发生的吻合口漏6例的防治临床资料。结果术后发生吻合口漏6例,吻合口漏的发生率4.37%,均经保守治疗痊愈。结论直肠癌保肛术后吻合口漏发病因素复杂,充分肠道准备、积极治疗原发病等可一定程度上预防其发生,抗感染、冲洗引流及TPN等保守治疗是其主要治疗手段。

Objective To investigate the prevention and cure of postoperation stomal leak of rectal cancel. Methods ： Analysing the stomal leak cases of the department of proctology Guanganmen hospital affiliated to China academy of Chinese medical sciences in 5 years and pertinent literatures. Results ： We performed 137 anterior resections of rectal cancer （ 112 patients used anastomats） in 5 years, 6 peoples underwent storeal leak（4.37% I. All the leakages were cured. Conclusion ： The cause of postoperation stomal leak of rectal cancer is complicated. Methods of prevention are good bowel preparation, treatment of complicating disease, no-tension anastomosis. The main methods are conservative therapies include anti-infection, douching and drainage, TPN.
