国产艾司洛尔治疗快速室上性心律失常疗效观察

Clinical study of effect of chinese Esmolol on supraventricular tachyarrhythmias

目的 为了进一步观察国产艾司洛尔治疗快速室上性心律失常的疗效及安全性。方法 110例快速室上性心律失常患者随机分为治疗组与对照组,治疗组55例予国产艾司洛尔1～3mg/kg静注,对照组55例予乙胺碘呋酮5～10mg/kg于15分钟静注;予艾司洛尔有效患者随机分为加服倍他乐克组及对照组(未加服倍他乐克)。结果 ①国产艾司洛尔组有效率7455%(41/55),乙胺碘呋酮组有效率8182%(45/55),两者比较无显著性差异(P>005);国产艾司洛尔与国外临床实验疗效相似(7455%VS84%P>005);②加服倍他乐克患者室上速复发明显低于对照组,二者比较有显著性差异(1905%VS6500%P<001);③艾司洛尔最主要不良反应为低血压,而乙胺碘呋酮为窦性心动过缓。结论 国产艾司洛尔治疗快速室上性心律失常安全、有效,加用长效β1受体阻滞剂口服可预防室上速复发。

Objective To further observe the efficacy and safety of Chinese esmolol on supracventricular tachyarrhythmias .Methods 110 patients were divided into therapeutic group and controlled group randomly .The former were treated with esmolol and the latter were trated with amiodarone .Esmolol and amiodarone were injected intravenously at doses of 1～3 mg/kg and 5～10 mg/kg respectively .The patients of therapeutic group who had achieved the therapeutic response were divided into oral metoprolol group and controlled group randomly .Results ①The effective rate of esmolol and amiodarone is 74 55% and 81 82% respectively .There was no significant difference between two groups( P >0 05);Also therapeutic effect of domestic esmolol was similar to the result obtained from foreign country clinical trial. ②The recurrence rate of oral metoprolol group was less than that of controlled group .There was significant difference between two groups (19 05% VS 65 00% P >0 01).③The most frequent adverse effect of esmolol observed during the study was hypotension and that of amiodarone was sinus bradycardia. Conclusion It is effective and safe that administering intravenous esmolol to patients with supraventricular tachyarrhythmias .And the majority of patients successfully treated with esmolol can be transferred to oral long-half betal-adrenergic blocking agents without the loss of the therapeutic response.
