主动电极右室不同部位起搏的长期动态观察 被引量:2

摘要 目的长期动态观察主动电极右室心尖部(RVA)与右室间隔部(RVS)起搏参数,探讨主动电极在右室间隔部的可行性、安全性、稳定性,以及右室不同部位起搏对心脏结构、左室射血分数(LVEF)、血清氨基末端脑钠肽前体(NT-proBNP)浓度影响。方法将69例植入双腔起搏器患者配对分为RVA组和RVS组,RVA组34例,RVS组35例,其中病态窦房结综合征33例,Ⅲ度房室传导阻滞36例。分别于术中即刻、术后1 d、1个月、12个月测量心室起搏阈值、电极阻抗和R波高度,术前、术后1个月、术后12个月测定心脏彩色多普勒超声心脏结构、LVEF、血清NT-proBNP浓度变化及观察心包压塞并发症。结果两组术中即刻电极阻抗较术后1 d、术后1个月、术后12个月均显著增高(P<0.05)。随访12个月后,两组间及组内比较起搏阈值、电极阻抗和R波高度差异无统计学意义(P>0.05);RVA组左心室舒张末内径较RVS组显著增大[(45.8±4.6)mm vs(41.8±3.0)mm,P<0.05];RVA组血清NT-proBNP浓度[(578.9±390.8)ng/mL]较RVS组[(492.2±108.6)ng/mL]显著升高(P<0.05);RVA组LVEF[(53.6±3.2)%]较RVS组[(62.3±2.5)%]显著降低(P<0.05)。长期动态观察提示RVA组血清NT-proBNP浓度较RVS组高,且与左室舒张末内径呈正相关(r=0.71,P<0.01),与左心射血分数呈负相关(r=-0.51,P<0.01)。两组均未发现心包压塞并发症。结论主动电极在RVA和RVS起搏均安全可行,长期动态观察显示RVS起搏在减少心脏重构、改善心功能方面优于RVA起搏。
