磁共振波谱分析在大脑皮层低级别胶质瘤诊断中的应用研究并文献复习

摘要 目的探讨磁共振波谱分析(MRS)在大脑皮层低级别胶质瘤中的诊断价值。方法回顾性分析我院5例经手术病理证实的大脑皮层低级别胶质瘤,均为男性,年龄19~70岁,平均48岁。所有病例均行MRI平扫和增强扫描,并行磁共振波谱分析。结果 5例均有局部脑回肿胀,邻近脑沟变浅或消失,3例发生在颞叶皮层,均无明显强化,其中1例出现囊性改变,2例发生在额叶皮层,轻度强化,其中1例伴有囊性变。5例波谱分析结果均有NAA峰降低,Cho升高,Cho/NAA比值明显增大,最小1.75,最大3.35。结论 MRS在大脑皮层低级别胶质瘤的诊断有重大价值,可作为和非肿瘤性病变鉴别的重要依据。 Objective To explore the clinical value of MRS in cerebral cortex low-grade gliomas.Methods MRI findings in 5 patients with pathologically proved cerebral cortex low-grade gliomas were analyzed retrospectively,5 male,mean age 48 years old.Results In all 5 cases,local cerebral convolution were swelling and adjacent anfractuosity were superficial or disappear,3 cases occurred in the temporal lobe cortex and had no enhancement with administration contrast agents,including one case occurred cystic changes,2 cases in the frontal cortex happened,mild strengthening,one with cystic changes.In 5 cases,the spectrum analysis results had NAA peak degraded and Cho peak rised,the ratio of Cho/NAA increased obviously,minimum was 1.75 and maximum was 2.35.Conclusion The clinical value of MRS in cerebral cortex low-grade gliomas is great.It is a useful indicator for differential diagnosis with nontumorous diseases
