经直肠超声在经尿道膀胱肿瘤电切术的应用价值

The study of transrectal ultrasonography using in transurenthral resection of bladder tumor

目的探讨经直肠超声(TRUS)在经尿道膀胱肿瘤电切术(TURBT)的应用价值。方法在膀胱适度充盈状态下,用7.0 MHz的探头对49例拟行TURBT术的膀胱肿瘤患者进行TRUS检查,重点观查肿瘤侵犯膀胱壁的情况,并与术后病理诊断比较,得出二者在膀胱肿瘤有无肌层浸润方面的符合率。结果本组49例患者中有30例TRUS显示肿瘤局限于黏膜层,有19例TRUS显示肿瘤浸润深度超出黏膜层(其中4例侵及浆膜层)。TRUS可较准确地判断膀胱肿瘤对膀胱壁有无肌层浸润,与术后病理诊断结果进行比较,符合率为85.7%。结论经直肠超声可较准确地判断膀胱肿瘤有无肌层浸润,对TURBT有重要的临床意义。

Objective To evaluate the applied value of transrectal ultrasonography（TRUS） in transurenthral resection of bladder tumor（TURBT）.Methods TRUS was performed in 49 patients with bladder tumor before their TURBT surgery.The depth of tumor invasion to the bladder wall was observed in the state of bladder moderate filling.The results of bladder tumor myo-level invasion detected by TRUS were compared with that of pathological diagnosis to analyze the coincidence rate of the two methods.Results In 30 patients the tumor invasions were limited to mucosa.In 19 patients the tumor invasions were beyond mucosa.TRUS can detect accurately whether the bladder tumor had invaded the muscular layer,the coincidence rate comparing with pathological diagnosis was 85.7%.Conclusions TRUS can assess effectively and accurately whether bladder tumor invaded the muscular layer and was helpful to TURBT in clinical using.
