经三角肌入路治疗移位肱骨外科颈骨折 被引量:5

摘要 目的探讨经肩峰前外侧三角肌劈开入路应用微创经皮钢板接骨技术(minimally invasive percutaneous plate osteosynthesis, MIPPO)治疗移位的肱骨外科颈骨折的可行性及临床效果。方法对2005年1月至2009年12月期间经三角肌劈开入路应用MIPPO技术治疗移位的肱骨外科颈骨折36例进行回顾性分析。根据Neer分型:累及肱骨外科颈的二部分骨折17例,三部分骨折13例,四部分骨折6例。结果术后切口均I期愈合,X线片显示所有螺钉位置良好,骨折复位满意。术后随访9—24个月,平均(13.75±4.72)个月;骨折愈合时间8—24周,平均(10.78±4.47)周;未发生腋神经损伤及肱骨头坏死和肩峰下撞击综合征。末次随访时肩关节活动范围:外展25°-90°,平均(81.53±12.35)°;前屈上举30°~180°,平均(146.11±35.52)°;外旋-10°-45°,平均(35.97±11.45)°;内旋达正0水平(T7~k)。Neer评分为60~95,平均(89.75±8.96);优26例,良6例,可1例,差3例;优良率为88.89%。肩关节外展功能与健侧比较,差异元统计学意义(P〉0.05)。结论经肩峰前外侧三角肌劈开入路应用MIPPO技术治疗移位的肱骨外科颈骨折,切口小,术后能早期进行功能锻炼,骨折愈合时间短,并发症少,是一种安全、微创、有效的治疗方法。
