MECT两种模式对精神分裂症疗效及记忆功能的影响

The efficacy of modified electro-convulsive treatment with two treatment models and the effects on memory function in patients with schizophrenia.

目的探讨MECT两种治疗模式(LOW0.5和DGX)对精神分裂症的疗效及记忆功能的影响。方法将100例精神分裂症患者随机分为两组,分别采用L0W0.5模式和DGX模式治疗,采用PANSS及WMS分别评定两组的疗效及记忆功能。结果 L0W0.5组总有效率为86%,DGX组总有效率为88%,两组无显著性差异(P>0.05);DGX组在第4次治疗后PANSS总分下降显著(P<0.05),L0W0.5组在第6次治疗后PANSS总分下降显著(P<0.05);L0W0.5组治疗前后WMS评分无显著性差异(P>0.05),DGX组在末次治疗后WMS评分比治疗前显著降低(P<0.05),但在末次治疗后第7天的各项分值即接近治疗前评分,到末次治疗后第14天时,各项分值均显著大于治疗前评分(P<0.05)。结论 MECT两种模式治疗精神分裂症的总体疗效相当,但L0W0.5模式起效慢,DGX模式起效快。L0W0.5组治疗前后记忆无明显影响,DGX组治疗后记忆下降明显(P<0.05),但在1～2周内逐渐得到恢复。

Objective To explore the efficacy and changes of memory function in patients with schizophrenia being treated with MECT under modes of either DGX or LOW 0.5.Method 100 patients diagnosed as schizophrenia according to CCMD-3 were randomly divided into two groups：group DGX（n=50） and group LOW0.5（n=50）.PANSS and WMS were used to evaluate efficacy and memory function before and after treatment.Results The total effective rate was 86% and 88% in group LOW0.5 and group DGX.There was no significantly difference between two groups（P0.05）;The PANSS total score decreased significantly after the fouth treatment in group DGX（P0.05）,but decreased significantly after the sixth treatment in group LOW 0.5（P0.05）;The WMS score was no significant difference before and after treatment in group LOW0.5（P0.05）.The WMS score in the last was significantly lower than before treatment in group DGX（P0.05）,but in 7 days after the last treatment,the score will be close to pre-treatment score,to 14 after the last treatment day,the scores were significantly higher than pre-treatment score（P0.05）.Conclusion There is no significant difference in clinical efficacy between DGX and LOW0.5 MECT in patients with schizophrenia.But slow onset LOW0.5 mode,DGX mode rapid onset.The memory before and after treatment and no significant effect in group LOW0.5 in group DGX,the memory decreased significantly after treatment,but gradually recover in 1~2 weeks.
