步长稳心颗粒对心力衰竭兔窦房结功能和心房肌电生理特性的影响 被引量:13

摘要 目的研究步长稳心颗粒对慢性心力衰竭(简称心衰)兔窦房结功能和心房肌电生理特性的影响。方法将32只日本大耳兔分为:假手术组、假手术稳心颗粒组、心衰组、心衰稳心颗粒组。采用主动脉瓣返流联合腹主动脉缩窄的方法制作心衰模型。假手术稳心颗粒组和心衰稳心颗粒组用稳心颗粒干预。8周后开胸测定兔窦房结功能,记录心房肌单向动作电位(MAP)及测定有效不应期(ERP)。结果与心衰组相比,心衰稳心颗粒组左室射血分数和左室短轴缩短分数显著增高,而左室收缩末及舒张末内径及容积均缩小,窦房结传导时间(SACT)缩短(P均<0.05),窦房结恢复时间(SNRT)、校正的SNRT(cSRNT)无差异。与假手术组比较,假手术稳心颗粒组和心衰组的SNRT、cSNRT、心房肌MAP时程及ERP均延长(P<0.05),但心衰组和心衰稳心颗粒组间并无差异(P>0.05)。结论稳心颗粒能改善心衰兔SACT,不加重兔心衰后窦房结功能低下;不影响心衰兔心房肌的MAP时程和ERP。 Objective To observe the effect of Wenxin Keli on sinusatrial node function and atrial electrophysiology in rabbits with congestive heart failure(HF). Methods HF was produced by combined aortic insufficiency and abdominal aortic constriction. 32 rabbits were randomly divided into four groups : sham-operated no-drug group, sham-operated Wenx- in Keli group, HF no-drug group and HF Wenxin Keli group. The rabbits in two groups were feed by Wenxin Keli. 8 weeks later, the sinusatrial node function was measured, atrial myocardium monophasic action potential duration (MAPD)and ef- fective refractory period (ERP) were record. The electrophysiologic parameters were evaluated by the technique of standard electric stimulation and MAP recording. Results Compared with HF group, the LVEF and LVFS were increased, the LVEDD,LVESD,LVEDV and LVEDS were decreased , SCAT was decreased in HF Wenxin Keli group. Wenxin Keli had no more negative effects on sinus node dysfunction in heart failure rabitts. Compared with sham-operated no-drug group, MAPD and ERP of atrial myocardium were prolonged in sham-operated Wenxin Keli group and HF group. The diffrence of MAPD and ERP of atrial myocardium between HF group and HF Wenxin Keli group was not significant. Conclusion Wenxin Keli can ameliorate abnormal SACT of atrial myocardium and do not have effect on MAPD and ERP of atrial myo- cardium in failing heart.
