浦东新区女性首发精神分裂症患者及时就诊的临床分析

The females of first Episode Schizophrenics had more problems than males to accept their routine treatments

目的:调查2007年1月1日至2008年12月31日浦东新区精神卫生中心门诊首次就诊精神分裂症患者(FES)的疾病未治疗时间(DUP,duration of untreated psychosis),分析其可能存在的社会心理影响因素。方法:采用自编的调查量表,将数据输入SPSS 11.0,采用参数及非参数统计方法进行分析。结果:共计43例首次发作的精神分裂症患者加入本试验,DUP平均(24.00±337.21)周,男(76.00±164.21)周,女(237.38±391.77)周。Pearson相关分析发现,DUP与就诊年龄呈显著统计学意义的正相关(r=0.404,P=0.007);同胞陪诊的患者DUP明显比直系亲属陪同的要长,差异有显著统计学意义(I-J=404.07,P=0.006)。将疾病未治疗时间(DUP)(周)按中位数24周为界分为长和短DUP两组(≤24周为短DUP组,>24周为长DUP组),长DUP组中女性比例明显增高,BPRS评分较低,差异有统计学意义(x2=4.532,P=0.033;t=2.537,P=0.015);风险因素分析显示,只有性别与DUP>24的相关性具有统计学意义(r=1.613,P=.037)。结论:女性、就诊时年龄偏大、同胞陪诊、精神症状不突出的精神分裂症患者的疾病未治疗时间(DUP)更长,即更难得到及时诊断和治疗。

Objective： To study the demographic characters of first episode schizophrenics who accepted their routine treatment in the clinics of Pudong Mental Health Centre from January 2007 to December 2008. Methods： All the patients were surveyed by the Self-edited Questionaire which included the general health state, diagnoses, career, family care, education etc. All the data were put in and analyzed with the software SPSSll. 0. Results： Altogether, 43 first episode schizophrenics （FES） were agreed to take part in the program. The average of the duration of untreated psychosis （ DUP）was 31.46 ± 10. 10 weeks（ Median ± SD）, the male was 76.00 ± 164.21 weeks,while the female was 237.38 ±391.77 weeks. There was a significant positive correlation between DUP and the age of visit（r =0.404, P = 0. 007 ） ;the DUP of FES who was accompanied by their siblings were much longer than by parents（ I -J = 404.07 ,p = 0. 006）. Longer DUP （ 〉 24weeks ）was independently associated with female and less scores of BPRS at entry （x2 = 4. 532 ,P = 0.033 ;t = 2. 537, P = 0.015 ）. The results of Logistic regression analysis was that only sex had the significant correlations with the Longer DUP（ r = 1. 613, P = 0. 037）. Conclusions： The DUP of female, older age of visit, accompanying person was siblings, and the less scores of BPRS （ Brief Psychiatric Rating Scale） was much longer than others.
