体外循环手术前后患者咽部菌群变化及术后切口细菌定植

Pre-and Post-CPB Throat flora and Post operative Wound Bacteria Colonization in Patients

心脏病手术患者术前 1天及术后 5天于咽部及手术切口区域采集标本 ,进行常规细菌培养和四区划线培养计数菌量。瓣膜置换术后患者 2 9例咽部菌群总量明显减少 (P <0 0 1) ,菌群构成比明显改变 (P <0 0 1) ,6例患者出现术后切口细菌定植 ,其中 5例为凝固酶阴性葡萄球菌 ;而先心病手术患者 4 2例咽部菌群总量和菌群构成比无明显变化 (P >0 0 5 ) ,17例患者出现术后切口细菌定植 ,其中 16例为凝固酶阴性葡萄球菌。体外循环手术后瓣膜置换手术患者咽部菌群明显变化 ,这种变化的机制及临床意义值得进一步研究。

To observe pre and post CPB throat flora and post operative wound bacterial colonization in patients.Cultures of the throat and operative site were done pre operatively and 5 day post operatively.Throat flora decreased and its consistent ratio changed in patients with prosthetic value replacement post CPB( P <0 01).Among them,there 6 patients with post operative wound bacterial colonization.On the other hand,there no obvious decrease of throat flora and its consistent ratio in patients with congenital heart defect CPB( P <0 05),However there were 17 patients with post operative wound bacterial colonization.Post CPB there were obvious changes of throat flora in patients with prosthetic value replacement.It is needed to study further for mechanisms and significance of the changes.
