高血压患者红细胞胰岛素酶活性与胰岛素抵抗

Relationship between erythrocyte insulinase activity and insulin resistance in patients with essential hypertension

摘要 探讨原发性高血压患者的红细胞胰岛素酶活性(EIA)与胰岛素抵抗(IR)的关系。方法用放射酶分析法检测40例原发性高血压患者及45例正常对照的EIA及相关指标.并计算胰岛素敏感性指数(ISI)。结果高血压患者的EIA、空腹血浆胰岛素(FINS)显著高于正常对照(P<0.01),而ISI显著低于正常对照(P<0.01)。伴2型糖尿病的高血压患者与不伴该病的高血压患者相比,前者的EIA、FINS高于后者,病程长于后者,而ISI低于后者(P<0.01或0.05)。相关分析表明,高血压患者的EIA与FINS、收缩压及病程呈显著正相关,而与ISI呈显著负相关。结论高血压患者的红细胞胰岛素降解速度显著高于正常,并与其IR的发生有关。 Objective To investigate the relationship between erythrocyte insulinase activity (EIA) and insulin resistance (IR) in patients with essential hypertension (EH). Methods EIA and related indices were determined and insulin sensitivity index (ISI) was calculated in 40 patients with EH and 45 healthy controls. ResuIts EIA and fasting plasma insulin (FINS) were increased and ISI was decreased in EH patients as compared with the controls. Moreover. in the EH patients with type 2 diabetes mellitus, EIA and FlNS were higher ; the duration of EH was longer and ISI was lower than those in the EH patients without type 2 diabetes. Correlation of EH, inversely correlated with ISI in EH patients. Conclusion Accelerated insulin degradation in erythrocytes exists in EH patients and plays some roles in the onset of IR in the EH patients. Besides, EH may be used as one index which can estimate the insulin sensitivity.
