加强下睑支持结构的下睑年轻化整形术

Rejuvenating lower blepharoplasty with repair of the supporting structure of the lower eyelid

目的 根据不同年龄及下睑老年化程度差异，提出相适应的下睑年轻化整形术。方法 对３５０ 例３０ ～６８ 岁下睑老年化者进行了加强下睑支持结构的整复术，根据个体差异，选择膨出眶脂或肥厚眼轮匝肌去除；眶隔筋膜及眼轮匝肌纵横重叠缝合；眶脂释放保留眶缘填充固定；眼轮匝肌瓣及外眦韧带悬吊固定以及膨体聚四氟乙烯眶缘凹陷充填等手术方式。结果 术后及随访结果表明，施行适应于不同个体的加强下睑支持结构整复术，年轻化明显，效果满意，并发症少。结论 下睑年轻化手术应以全面加强下睑支持结构为基础。

Objective To find proper rejuvenating lower blepharoplasty in accordance with different ages and aging degrees of the lower eyelid.Methods The lower blepharoplasty by repairing the supporting structures was performed in 350 cases at the age of 30 to 68. According to the individual differences, there were adopted operations such as removal of protruding orbital fat and excess orbicular muscle, overlapping suture of the orbital septum and orbicular muscle, arcus margin release and reposition of orbital fat, suspension of the orbicular muscle flap and lateral canthal ligament and augmentation with PTFE for orbital cavity. Results Follow￣ups to the operation showed that all the above mentioned treatments were excellent, the patients being satisfied with looking younger with no complications. Conclusions The rejuvenating lower blepharoplasty should be based on all￣round repair of the supporting structures of the lower eyelid. The key to obtain satisfactory results is considered to be proper adoption of operative methods corresponding to individual differences.
