膀胱移行细胞癌p21^(WAF1/CIP1)蛋白表达与临床病理的相关研究

Expression of p21^( WAF1/CIP1) and its Correlation with Clinicopathological Features in Human Bladder Transitional Carcinoma

目的：研究细胞周期蛋白依赖性激酶抑制因子（ｃｙｃｌｉｎｄｅｐｅｎｄｅｎｔｋｉｎａｓｅｉｎｈｉｂｉｔｏｒ，ＣＤＫＩ）ｐ２１ＷＡＦ１／ＣＩＰ１在膀胱移行细胞癌中表达情况，并探讨ｐ２１ＷＡＦ１／ＣＩＰ１表达与膀胱移行细胞癌临床病理指标的相关性。方法：应用Ｓ－Ｐ免疫组织化学方法检测９１例膀胱移行细胞癌（其中包括ＴａＧ１期膀胱移行细胞癌１５例）ｐ２１ＷＡＦ１／ＣＩＰ１蛋白表达情况，并探讨ｐ２１ＷＡＦ１／ＣＩＰ１表达与膀胱移行细胞癌的分化程度、浸润深度、淋巴结转移、ＴＮＭ分期等的相关性。结果：膀胱移行细胞癌ｐ２１ＷＡＦ１／ＣＩＰ１阳性表达率为４８．４％（４４／９１）；其中高、中分化的膀胱移行细胞癌ｐ２１ＷＡＦ１／ＣＩＰ１阳性表达率为５８．５％（３８／６５），低分化为２３．１％（６／２６），组间差异显著（Ｐ＜０．０５）；ｐ２１ＷＡＦ１／ＣＩＰ１阳性表达率ＴａＧ１期膀胱移行细胞癌为９３．３％（１４／１５），Ｔ１＋Ｔ２期为５２．１％（２５／４８），Ｔ３＋Ｔ４期为１７．９％（５／２８），３组间差异显著（Ｐ＜０．０５）；ＴＮＭ分期Ⅰ～Ⅱ期膀胱移行细胞癌ｐ２１ＷＡＦ１／ＣＩＰ１阳性表达率为５７．４％（３５／６１），Ⅲ～Ⅳ期为３０．

Objective:This paper was designed to study the expression of p21 WAF1/CIP1 and its correlation with clinicopathological features in human bladder transitional carcinoma. Methods:The cyclin dependent kinase inhibitor(CDKI) p21 WAF1/CIP1 was detected by S P immunohistochemic method in 91 cases of paraffin embedded human bladder transitional carcinoma, including 15 cases of TaG1 stage of bladder transitional carcinoma. Results: The general positive expression rate of p21 WAF1/CIP1 was 48.4%(44/91) for all the 91 cases of bladder transitional carcinoma. The positive expression rate of p21 WAF1/CIP1 showed significant difference between the well, moderately differentiated group (58.5%, 38/65) and the poorly differentiated group (23.1%, 6/26) of bladder transitional carcinoma (P<0.05). The positive expression rate of p21 WAF1/CIP1 , which was 93.3%(14/15) for TaG1 group, 52.1%(25/48) for T 1+T 2 group and 17.9%(5/28) for T 3+T 4 group, respectively, showed significant difference among the three groups staged by the invasion depth (P<0.05). The positive expression rate of p21 WAF1/CIP1 of Ⅰ～Ⅱ stage (according to TNM staging method) of bladder transitional carcinoma was 57.4%(35/61),which was significantly different from that of Ⅲ～Ⅳ stage (30.0%,9/30). Conclusion: The positive expression rate of p21 WAF1/CIP1 was higher for the well-differentiated and non-invasive bladder transitional carcinoma. The expression of p21 WAF1/CIP1 might be of a certain value for the prognosis of bladder transitional carcinoma.
