摘要
目的改良的平产消毒、检查、会阴侧切方法对医院感染的影响。方法 2005年1月-2010年12月14 635例采用改良的消毒、检查方法平产分娩产妇为观察组,其中会阴侧切9982例,从中抽检空气样本50例检测细菌菌落数;1999年1月-2004年12月11 873例平产分娩产妇为对照组,其中会阴侧切7836例;以《全国医院感染诊断》为诊断标准,比较两组产妇医院感染率、新生儿感染率、伤口甲级愈合率等指标。结果观察组产妇医院感染率、产妇会阴伤口感染、新生儿感染及其他感染的发生率均低于对照组(P<0.01);切口甲级愈合率99.6%,高于对照组(P<0.01)。结论改良的消毒、检查、会阴侧切方法可有效降低医院感染,因此可以替代传统的平产消毒、检查、会阴侧切方法。
OBJECTIVE To observe the influence of improved sterilization, examination,episiotomy methods during normal delivery on nosocomial infection. METHODS The experimental group was the 14635 cases women who experienced modified disinfection and examination method in our hospital from Jan 2005 to Dec 2010, in which 9982 cases experienced episiotomy,and we selected 50 cases air samples randomly to detect their bacterial colonies from experimental group. The control group was 11873 cases women who experienced normal labor and delivery in our hospital from Jan 1999 to Dec 2004, in which 7 836 cases experienced episiotomy. Two groups base on national nosocomial infection diagnosis as the diagnostic criteria to compare their hospital infection rates, maternal and neonatal infection rates, class A rates of wound healing and biological monitoring. RESULTS Hospital infection rate of the experimental group was significantly lower than the control group(P〈0.01), maternal perineal wound infection rates, neonatal infection rates and other infection rates was also lower than the control group(P〈0.05), class A rates of wound healing 99. 6% was significantly higher than the control group(P〈0. 01), biological monitoring of equipment surface, air and the hands of staff in the experimental group was better than in the control group(〈P〈0.01). CONCLUSION The improved method of normal delivery sterilization, examination, episiotomy can effectively reduce the rate of nosocomial infection, so it can replace the traditional method, examination and episiotomy.
