应变和应变率成像结合冷加压试验对2型糖尿病患者颈总动脉应变及应变率储备的研究

摘要 目的探讨应用应变和应变率成像结合冷加压试验评价2型糖尿病患者颈总动脉应变及应变率储备的可行性及其临床价值。方法选取56例无血管并发症的2型糖尿病患者作为病例组,63名健康志愿者作为对照组。全部进行应变和应变率成像及冷加压试验,获得冷加压试验前后颈总动脉的最大圆周应变和应变率,计算圆周应变和应变率储备。对病例组和对照组的圆周应变和应变率储备进行对比分析。结果病例组的颈总动脉圆周应变(18.16%±4.62%vs.28.56%±7.42%,P<0.05)和应变率储备(20.33%±5.81%vs.32.09%±6.01%,P<0.05)均低于对照组,两组比较差异有统计学意义。结论应变和应变率成像结合冷加压试验可以获得颈总动脉的圆周应变和应变率储备,圆周应变和应变率储备可作为评价2型糖尿病患者早期动脉壁机械运动特性改变的新指标,同时可以反映颈总动脉的内皮功能。 Objectives To investigate the feasibility and clinical value of strain and strain rate imaging combined with cold pressor test (CPT) in assessing reserve of circular strain (RCS) and circular strain rate (RCSR) of common carotid artery in patients with type 2 diabetes. Methods Fifty six patients with type 2 diabetes without vascular complications were involved into our study as case group. Sixty three healthy volunteers were supplied as normal control group in the same period. Strain and train rate imaging combined with CPT were carried on in these two groups. Maximum of circular strain (CSmax) and circular strain rate (CSRmax) of common carotid artery were measured, which were supplied by automatic analysis software. RCS and RCSR were calculated, and comparative analysis was performed. Results RCS (18.16%±4.62% vs. 28.56%±7.42%,P〈0.05) and RCSR (20.33%±5.81% vs. 32.09%±6.01%,P〈0.05) of common carotid artery in case group were less than those in control group, and the differences were significant (P〈 0.05). Conclusions RCS and RCSR of common carotid artery could be gained through strain and strain rate imaging combined ned.RCS and RCSR could be new indexes in assessing early change of mechanical movement of common carotid artery in patients with type 2 diabetes, and could represent endothelial function of common carotid artery.
