老年与青年结核性胸膜炎不同临床特点分析

Clinical features of tuberculous pleurisy in elderly and young people

目的分析老年结核性胸膜炎的临床特点。方法南阳市结核病防治所2009年3月至2011年3月共收治老年结核性胸膜炎患者54例,青年结核性胸膜炎52例,就两组临床特点、实验室检查资料进行对比分析。结果老年患者以气急、胸痛症状多见,青年患者发热明显。老年组血性胸腔积液、合并症和不良反应例数明显高于青年组(P<0.05),治疗后胸腔积液吸收时间亦长于青年组(P<0.05)。结论老年结核性胸膜炎临床表现多样,因伴有老年常见合并症易漏诊、误诊。掌握老年患者的临床特点,综合分析,早期诊断,治疗上除常规抗痨外,应加强全身支持疗法,采取多种措施,提高治愈率。

Objective To analyze the clinical features of tuberculous pleurisy in elderly and young people.Methods Fifty-four old patients with tuberculous pleurisy and fifty-two young patients with tuberculous pleurisy were treated in Nanyang tuberculosis control in March 2009-March 2011.Making a comparative analysis on the clinical features of the two groups.Results Elderly patients with shortness of breath and chest pain symptoms are more common,and young patients with fever are common.The number of cases of bloody pleural effusion,complications and adverse reaction in the elderly group was significantly higher than those the young group（P0.05）.The absorption time of pleural effusion in the elderly group are longer than that of the young group（P0.05）.Conclusions The clinical manifestations of old tuberculous pleurisy are diverse.Because of complications,it is easily misdiagnosed.Master the clinical characteristics of elderly patients and then make a comprehensive analysis,early diagnosis.Besides the conventional anti-tuberculosis,general supportive therapy should be strengthened.By taking all kinds of measures,the curative rate can be improved.
