摘要 目的比较分析良性前列腺增生3种微创手术,探讨最佳手术方式。方法回顾分析1998年5月至2008年4月TURP术652例,2008年5月至2010年8月TUPKP术162例。复习钬激光治疗良性前列腺增生文献资料,将三者进行比较。结果TURP与TUPKP术后,总体并发症TURP组大于TUPKP组(P<0.05);钬激光手术时间、术中冲洗液量大于TURP组,而术中出血量、术后出血量、术后膀胱冲洗时间、术后留置导尿管时间及术后并发症小于TURP组(P<0.05)。钬激光组的出血量较TUPKP组则明显减少(P<0.05)。在术后保留导尿时间、膀胱持续冲洗时间方面,钬激光组有明显优势。结论钬激光及等离子治疗良性前列腺增生方面有望替代TURP和开放手术,成为良性前列腺增生手术的主要手术方式。 Objective To study the best surgical approach in the three types of minimally invasive surgery for treating benign prostatic hyperplasia.Methods The clinical data of BPH were reviewed retrospectively.652 cases and 162 cases with BPH were treated with TURP snf TUPKP,respectively,then their clinical data were compared to the cases treated with holmium laser treatment.Results The overall complications of TURP were more than TUPKP(P0.05);The surgery time,intraoperative fluid volume in holmium laser group were more than those in TURP group,but blood loss,postoperative bleeding,postoperative bladder washing time,postoperative catheterization time,postoperative complications were less than TURP(P0.05).Blood loss was significantly less in the holmium laser than in the TUPKP group(P0.05).Conclusion The holmium laser and TUPKP of BPH are superior toTURP and open surgery.
