巨大儿脐血脂联素水平及相关因素分析

Measurements of umbilical cord blood adiponectin level of macrosomia and analysis of related factors

目的探讨巨大儿脐血脂联素水平变化并分析其相关因素。方法研究对象为巨大儿及其产妇50例，正常体重儿及其产妇50例，应用酶联免疫吸附法测定新生儿脐血及产妇血脂联素水平，用免疫比浊法测定三酰甘油、总胆固醇，分析新生儿脐血脂联素与母血脂联素水平、出生体质量、出生身长、体质量指数、宫高、腹围、血脂水平的相关性。结果①巨大儿脐血脂联素水平高于正常体重儿，差异有统计学意义（P〈0．05）。②尚不能认为巨大儿脐血脂联素水平与母血脂联素水平相关（P〉0．05）。③新生儿脐血脂联素水平与出生体质量、出生身长、体质量指数、胸围、腹围呈正相关（P〈0．05），与胆固醇、甘油三酯水平无相关性（P〉0．05）。结论脐血脂联素水平与巨大儿的发生有关，测定脐血脂联素水平有助于判断巨大儿的发展趋势并有助于预防肥胖相关性疾病，加强孕期监护可预防巨大胎儿的发生。

Objective To investigate the umbilical cord blood adiponectin levels of macrosomia and analyze the related factors. Methods In the present study, 50 normal weight infants and 50 macrosomia enrolled in puerpera and cord plasma adiponectin were determined by enzyme linked immunosorbent assay（ ELISA）, serum triacyl- glycerol ganglion （TG）, total cholesterol （TCH） were assayed with turbidimetry. And the correlation between cord plasma adiponectin and adiponectin, maternal weight, height, the gestational weeks at delivery, maternal abdominal circle, birth weight, body mass index（ BMI）, and blood lipids were analyzed. Results （1）Compared with normal weight infants, the umbilical cord blood adiponectin levels in macrosomia increased significantly （ P 〈 0. 05 ） ; （2）There was no difference between cord plasma adiponectin in fetal macrosomia and puerpera adiponectin （P 〉 0.05 ） ;（3）The adiponectin levels of fetal macrosomia were positively correlated with birth weight, height, abdominal circumference and chest circumference （P 〈 0.05 ）. Conclusions Adiponectin levels in umbilical cord blood is correlated with macrosomia. It may help to estimate the occurrence of macrosomia and to prevent obesity related diseases. The antenatal care is reinforced during the pregnancy to prevent occurrence of macrosomia. The maternal adiponectin levels, weight, height, the gestational weeks at delivery, maternal abdominal circle are predictive factors for diagnosis of macrosomia.
