地特胰岛素和二甲双胍对血浆C反应蛋白和血脂的影响

Effects of insulin detemir plus metformin on C - reactive protein and blood fat

目的分析地特胰岛素联合二甲双胍对初诊2型糖尿病患者血浆c-反应蛋白（CRP）和血脂的影响。方法选取新诊断2型糖尿病患者100例进行地特胰岛素起始剂量10u与二甲双胍0．5g，3次／d联合治疗，疗程为12周，另入选100例同期进行健康体检的正常人群为正常对照组，观察两组研究对象基线血糖指标，血脂以及炎症因子CRP差异，并比较2型糖尿病组治疗前后血糖指标、血脂和CRP的变化。结果治疗前2型糖尿病组的血糖指标、血脂及cRP明显高于对照组，其他基础临床资料比较差异无统计学意义（P〉0．05）。治疗12周后2型糖尿病组的血糖指标、血脂和血浆CRP浓度，较治疗前明显下降（P〈0．01）。结论地特胰岛素与二甲双胍联合治疗可以改善初诊2型糖尿病患者的糖代谢，降低患者血浆CRP的浓度，降低胆固醇、甘油三酯和低密度脂蛋白。

Objective To observe the effects of insulin detemir plus metformin on C - reactive protein and blood fat in the treatment of patients with newly diagnosed type 2 diabetes. Methods One hundred patients with newly diagnosed type 2 diabetes were given insulin detemir （beginning dose 10 U/d and metmormin 0. 5 g, tid for 12 weeks;while 100 cases selected for the same period from the normal healthy population were taken as control group. Both groups＇ baseline blood glucose, blood fat and C -reactive protein were observed. Results Before treatment, blood glucose targets, blood fat and C -reactive protein in type 2 diabetes group were obviously higher than those in control group, but there were no difference in other clinical datas. After 12 weeks＇ treatment, in type 2 diabetes group, there was a significant decreased in blood glucose targets, blood fat and C - reactive protein （P 〈 0. 01 ）. Conclusions Insulin detemir and mefformin combined therapy can improve glucosed metabolism in patients with newly diagnosed type 2 diabetes, and decrease the plasma concentration of CRP. TC. TG and LDL - C.
