类风湿性关节炎合并甲状腺非霍奇金氏淋巴瘤1例报道并文献复习

Primary non-Hodgkin's lymphoma of thyroid gland accompany with rheumatoid arthritis:a rare case report and literature review

目的:提高对类风湿性关节炎合并非霍奇金氏甲状腺淋巴瘤临床和病理特征的认识。方法:复习类风湿性关节炎合并非霍奇金氏甲状腺淋巴瘤临床资料文献并分析1例患者的临床过程。结果:患者确诊类风湿性关节炎治疗15年,病情稳定后发现双侧甲状腺肿大弥漫性病变,手术病理确诊为甲状腺非霍奇金弥漫大B细胞淋巴瘤(DLBCL),经R-CHOP方案化疗6个周期并给予局部放疗后病情稳定。结论:类风湿性关节炎合并非霍奇金氏甲状腺淋巴瘤的病例正在增加,两者可能有相同的发病机制,早期发现,预后好。

Objective：To increase the clinical and pathological features awareness of primary non-Hodgkin＇s lymphoma of thyroid gland accompany with rheumatoid arthritis（RA）.Methods：We analyzed the clinical course of a case with primary non-Hodgkin＇s lymphoma of thyroid gland accompany with RA and reviewed related literature.Results：The patient＇s conditions has been stabilized since being diagnosed with RA fifteen years ago,after which she was observed with diffuse enlargement of the thyroid gland.Four months later,she was diagnosed as diffuse large B cell non-Hodgkin＇s lymphoma（DLBCL）by postoperative pathological examination.Six cycles of R-CHOP chemotherapy and local radiotherapy was given to the patient,and now the condition is stable.Conclusion：The patients of primary non-Hodgkin＇s lymphoma of the thyroid gland accompany with RA are increasing.The pathogenesis of RA and non-Hodgkin＇s lymphoma may be similar in some respect.Earlier diagnosis can get better prognosis.
