介入溶栓导管在急性下肢动脉血栓形成中的应用

Catheter-directed Thrombolysis for Acute Lower Extremity Arterial Thrombosis

目的评价介入溶栓导管溶栓治疗在下肢动脉血栓中的临床应用价值。方法 20例下肢动脉血栓患者,经动脉内介入溶栓导管脉冲式推注协同持续灌注泵入尿激酶溶栓治疗,观察肢体血管复通情况。结果治疗后复查彩色多普勒,患者24h内完全复通8例,48h内溶通6例,部分溶通4例,但彩超评估血栓平面均有所下降,患者疼痛明显减轻或消失。结论介入溶栓导管溶栓,定位准确,实现了均匀溶栓,疗效肯定,是一种较为理想的治疗方法。

Objective To evaluate the clinical value of catheter-directed thrombolysis in the treatment of acute lower extremity arterial thrombosis.Methods Twenty patients with lower extremity arterial thrombosis underwent catheter-directed thrombolysis with pulse-spray injection and continuous infusion of urokinase.The recanalization situation was observed in all patients.Results Color Doppler ultrasound showed complete recanalization within 24 hours in 8 patients,complete recanalization within 48 hours in 6 patients,and partial recanalization in 4 patients.The location of thrombi declined and patients＇ pain significantly reduced or disappeared.Conclusion The catheter-directed thrombolysis is effective and desirable for acute lower extremity arterial thrombosis with the advantages of accurate location and uniform thrombolysis.
