伴三系病态造血初发性急性髓细胞白血病患者的临床及实验室特征分析

摘要 目的探讨急性髓细胞白血病(AML)的病态造血特点、免疫表型与临床疗效的关系。方法对87例AML患者骨髓细胞学特点、免疫分型及临床疗效进行比较分析。结果 87例AML患者中发现12例(13.8%)伴有三系病态造血(AML-TMD),且属于M3以外的法、美、英协作组诊断标准的各亚型。AML-TMD组骨髓原始细胞计数、骨髓细胞免疫分型表达异常:主要为跨系CD33与CD19/CD7/CD38共表达(37.1%)与跨阶段CD34与CD11b共表达(43.2%),与无TMD的AML组异常表达(12.1%、14.7%)比较差异有统计学意义(P<0.05);且AML-TMD首次完全缓解率低于无TMD的AML组,两者差异有统计学意义(P<0.05)。结论 AML-TMD治疗后各系的病态造血并不消失,对化疗不敏感,首次完全缓解率低,预后差。
