封闭式负压引流技术在外科难治创面治疗中的应用

目的:观察封闭式负压引流技术(VSD)治疗56例外科难治创面的疗效。方法:对近3年来收治的56例外科难治创面,采用多聚乙烯醇明胶海绵材料覆盖封闭,24小时间断负压吸引(维持负压在60～80kPa)。结果:38例经1次VSD技术引流7天,12例经两次VSD技术引流14天,6例经3次引流21天,无感染创面引流后未发生感染,感染创面感染得到控制,创面缩小,肉芽组织新鲜,部分创面直接愈合,2例游离植皮及1例皮瓣转位因术前创面有感染术后周边少许坏死渗出,与换药后愈合,其余创面Ⅱ期缝合或植皮治疗后愈合。患肢功能及外观恢复良好,与对照组比较,在住院天数、创面愈合率、换药次数及治疗总费用等方面差异均有统计学意义(P<0.05)。结论:VSD技术对于外科难治创面的治疗效果好。

Objective：observing the enclosed the negative pressure drainage technology（VSD）56 cases of surgical treatment to cure the curative effect of the wound.Methods：for 3 years of our 56 cases were difficult to cure the surgical wound polyvinyl alcohol（pva）gelatin sponge materials（brand name,wei tong）cover closed,24 hours continuous negative pressure attract（keep negative pressure in 60～80 kPa）.Results：by 38 cases a VSD technology drainage 7 days,12 patients through two VSD technology drainage 14 days,6 patients with three times drainage 21 days,no wound infection after an infection,drainage not infected wound infection control,wounds,get narrow granulation tissue,wound healing of fresh direct,2 cases of 2 cases and 1 case with a flap transfer for preoperative wound infection around a little necrosis after the change,and after the second phase wound healing,the skin graft or after treatment healing.Limb function and appearance good recovery,compared with controls,days in the hospital,wound healing rate,treatment and total number of treatment costs are statistically significant difference（P0.05）.Conclusion：VSD technology for surgical difficult to wound healing.
