胱抑素C在糖尿病性肾病中评估肾小球滤过率的价值研究 被引量:2

摘要 目的比较不同阶段2型糖尿病肾病患者血清胱抑素C(CysC)水平,探讨相关性。方法将柳州医学高等专科学校第一附院收治的2型糖尿病患者148例,按内生肌酐清除率Ccr值分为三组,测定各组CysC水平、血肌酐(Scr),计算内生肌酐清除率(Ccr)、C-G公式计算的肾小球率过滤(CG-GFR)。结果研究组各组间血清胱抑素C有显著差异。以Ccr 90ml/(min.1.73m2)或60ml/(min.1.73m2)为切点作ROC曲线,三者之中,CysC对Ccr的诊断效能最强,CG-GFR次之,血肌酐最弱。结论血清CysC水平的升高与糖尿病肾病密切相关,CysC比血肌酐、CG-GFR可更敏感地评估早中期肾功能损害的改变。 Objective To compare the levels of serum Cysytatin C(CysC) on the different stages of patients with type 2 diabetic nephropathy and study its correlation.Method 148 patients with type 2 diabetes were divided into three groups regarding the values of Ccr.CysC and Scr were detected,Ccr and the CG-GFR were calculated.Results The CysC levels of the three groups of were significantly different.Using Ccr 90 ml/(min·1.73 m2) or 60 ml/(min·1.73 m2) as the cut-point for the ROC curve,the diagnostic efficacy of CysC to Ccr was the best,Scr was the weakest.Conclution The plasma CyC level is increased with diabetic nephropathy.Compared with Scr or CG-GFR,CysC can be more sensitive to assess the renal function changes in diabetic nephropathy.
