桂林市≥15岁城乡居民精神疾病流行病学调查

EPIDEMIOLOGICAL SURVEY OF MENTAL DISORDERS IN URBAN AND RURAL IN GUILIN,CHINA

目的:了解桂林市≥15岁城乡居民各类精神疾病的患病水平和分布特征。方法:2007年7～12月采用多阶段分层整群抽样方法随机抽取桂林3个城区和3个县4 410名≥15岁的居民为调查对象进行入户调查。以中国疾病预防控制中心精神卫生中心提供的中文版的复合性国际诊断问卷(CIDI3.0)为筛查工具,以国际疾病及相关健康问题分类第10版(ICD-10)为诊断标准。结果:完成调查3 783人,调查完成率为85.78%。精神疾病总时点患病率和总终生患病率分别为2.54%和2.96%。按终生患病率高低排列,桂林市前三位特定精神疾病是精神分裂症(0.93%)、酒精依赖(0.90%)和重性抑郁(0.66%)。总终生患病率,城市(3.03%)高于农村(2.93%),但差异无统计学意义(P=0.867);女性(2.40%)低于男性(3.56%),差异有统计学意义(P=0.037);壮族(6.15%)高于汉族(2.63%),差异有统计学意义(P=0.000)。结论:精神分裂症、酒精依赖和重性抑郁是桂林地区最常见的精神疾病,已成为重大的公共卫生问题;农村、男性和壮族人群的精神疾病患病率较高,应针对其开展防治与康复研究工作。

Objective：To investigate the prevalence and distribution of mental disorders among people aged more than 15 in urban and rural in Guilin.Methods：From July,2007 to December,2007,4 410 subjects aged more than 15 were randomly selected for 3 urban areas and 3 counties by stratified multi-stage cluster sampling method.All subjects were investigated by face to face interview.The Chinese version of Composite International Diagnostic Interview（CIDI3.0） Provided by the Chinese Center for Disease Control and Prevention was used as the screening tool and the diagnoses were made with the 10th edition of international classfication of Disease Criternia.Results：A total of 3 783 subjects completed the screening,the completion rate was 85.78%.The general point prevalence was 2.54%,and the general lifetime prevalence was 2.96%.Ordered by lifetime prevalence rate,the most common specific disorders were major Schizophrenia（0.93%）,alcohol dependence disorder（0.90%） and depression disorder（0.66%）.The general lifetime prevalence was 3.03% in urban and 2.93% in rural（P=0.867）,3.56% in men and 2.40% in women（P=0.037）,and 6.15% in Zhuang and 2.63% in Han（P=0.000）.Conclusion：Schizophrenia,alcohol dependence disorder and depression disorder are the most common mental disorders and have become the public health problem in Guilin.To do this,it is necessary to carry out targeted research in prevention and rehabilitation.
