联机血液透析滤过对维持血液透析患者血TGF-β1、CRP的影响 被引量:4

摘要 目的探讨联机血液透析滤过(On-line Hemodiafiltration,联机HDF)清除维持血液透析患者血转化生长因子β1(Transforming growth factor beta 1,TGF-β1)、C反应蛋白(C-reactive protein,CRP)的有效性和安全性。方法将66例维持血液透析患者随机分为三组,每组22例,分别进行联机HDF、高通量血液透析(HPD)和常规血液透析(CHP)治疗,观察3个月。选取20名健康体检者为正常对照组。透析前和每月透析后采患者静脉血测定TGF-β1、CRP水平,以观察不同透析方式对细胞因子的清除效果。同时比较患者血TGF-β1水平与血CRP水平的相关性。结果各组维持血液透析患者的血TGF-β1、CRP水平均高于正常对照组(P<0.01);联机HDF组透析后血TGF-β1、CRP水平较透析前明显下降(P<0.0 1);患者血TGF-β1水平与血CRP水平呈显著正相关(P<0.01)。结论联机HDF对毒素清除较其他透析方式更安全有效,是尿毒症患者的最佳透析方式,值得推广应用。 Objective To investigate the effectiveness and safety of online hemodiafiltration(HDF) on Transforming growth factor beta 1(TGF-β1) and C reactive protein(CRP) in maintenance hemodialysis patients. Methods Sixty-six patients with maintenance hemodialysis were divided into three groups: the online hemodiafiltration group,the high flux hemodialysis(HPD) group and the conventional hemodialysis(CHP)group.These groups were observed for 3 months.20 healthy persons were selected as the normal control group.Compare the difference of levels of TGF-β1 and CRP in maintenance hemodialysis patients before and after the treatment of hemodialysis to observe the removing effect of different dialysis for cytokines.Correlation between levels of TGF-β1 and levels of CRP in patients were also investigated. Results The serum TGF-β1 and CRP levels of dialysis groups were significantly higher than those in the normal control group(P〈0.01).After 3 months'treatment,the serum levels of TGF-β1 and CRP in the HDF group were lower than before(P〈0.01).The relation between serum TGF-β1 levels and the serum CRP levels was positive(P〈0.01). Conclusion Online hemodiafiltration is more safe and effective than the other hemodialysis in removing toxin in maintenance hemodialysis patients.It's one of the best dialysis ways for patients with ESRD and is worth promoting.
