摘要
目的:评价数字减影血管造影(DSA)与磁共振血管成像(MRA)在大脑中动脉(MCA)狭窄诊断中的应用价值。方法:对62例脑缺血疾病患者1周内进行DSA和MRA检查,并且比较其影像学结果。结果:62例病人共124条MCA中,MRA及DSA均显示无狭窄的78条,狭窄<10%的5条,狭窄10%~50%的5条,狭窄51%~99%的6条,闭塞的10条。MRA对无狭窄的敏感性、特异性分别为92.86%、86.96%,狭窄<10%的敏感性、特异性分别为85.00%、97.79%,狭窄10%~50%的敏感性、特异性分别为87.56%、95.83%,狭窄51%~99%的敏感性、特异性分别为88.56%、96.58%,闭塞的敏感性、特异性分别为100.00%、100.00%。结论:MRA能较为准确地反映MCA狭窄情况,可以在结合临床症状的前提下作为筛查MCA狭窄的首选手段。
Objective: To evaluate the value of DSA and MRA in middle cerebral artery(MCA) stenosis. Methods: 62 patients with cerebral ischemic disease within a week underwent DSA and MRA,and the imaging results were compared. Results: In MRA and DSA of 62 patients a total of 124 middle cerebral arteries,78 showed no stenosis,5 had less than 10% stenosis,5 had 10% to 50% stenosis,6 had 51 % to 99% stenosis,10 were completely occluded.The sensitivity and specificity of MRA for those without stenosis were 92.86%,86.96%; when stenosis developed 10%,sensitivity and specificity were 85.00%,97.79%;when it evolved to 10% to 50%,sensitivity and specificity were 87.56%,95.83%;stenosis of 51% to 99% saw a 88.56% sensitivity and 96.58% specificity;finally,both sensitivity and specificity hit 100.00% when occlusion occurred.Conclusion: MRA,a more accurate response to MCA stenosis,can be the first choice in screening MCA stenosis when combined with clinical symptoms.
