玻璃体切除术中刮除角膜上皮的三类人群术后疼痛程度的比较

Assessment of post-vitrectomy pain of three groups patients with corneal epithelium removal.

目的对玻璃体切除术中刮除角膜上皮的3类人群的术后疼痛程度进行比较。方法对玻璃体切除术中必须刮除角膜上皮的眼外伤18例（18眼）、糖尿病23例（23眼）、无眼外伤无全身病者20例（20眼）在术后4h、8h、12h、24h用视觉模拟评分法（VAS）进行术后疼痛程度的比较。结果术后4h眼外伤患者平均疼痛评分为5．51±2．13，糖尿病患者为3．96±2．42，无眼外伤及全身病者平均疼痛评分为4．73±2．53，差异有统计学意义；术后12h分别为4．82±2．16、3．30±2．12和4．35±2．48，差异有统计学意义；术后24h分别为4．16±2．32、3．11±2．04和3．85±2．36，差异无统计学意义。结论刮除角膜上皮的3个组中，术后12h以内，眼外伤组和无眼外伤及全身病组疼痛明显强于糖尿病组，术后24h及48h3组疼痛程度无明显差异。

Objective To observe and assess post-vitrectomy pain of three groups patients with epicorneal removal. Methods Patients with corneal epithelium removal in vitrectomy surgery were asked pain responses with visual analog scale（VAS） in the different postoperative time. The three groups included 18 cases of ocular injury, 23 cases of diabetic retinopathy and 20 patients without eye injury or general disease. Those items were analysed in 4 h, 8 h, 12 h and 24 h after surgery. Results The mean post-operative pain score was （5.51 ± 2.13） 4 h after surgery in ocular injury group, （3.96 ±2.42） in diabetic retinopathy group, and （4.73 ±2.53） in the group of without ocuar injury or general diseases. There were statistical significace between each other in the three groups. It was （4.82 ± 2.16 ）, （3.30± 2.12 ） and （4.35± 2.48） respectively in the three groups 12 h after surgery. And statistical significance was identified. It was （4.16 ±2.32）, （3.11 ±2.04） and （3.85 ±2.36） 24 hours after surgery, the difference have no statistial significant. Conclusion After corneal epithelium removal, the pain of patients with eye injury and patients without eye injury or general disease are worse than diabetic patients before 12 hours postoperatively, but they have no difference after 24 hours.
