BNP、MYO、cTnI三项联合测定对AMI早期诊断的临床意义

摘要 通过采用化学免疫发光法定量检测47例急性心肌梗死(AMI)患者的BNP、MYO、cTnI并进行对照分析,探讨AMI患者的血浆脑钠肽(BNP)、肌红蛋白(MYO)、肌钙蛋白cTnI的浓度变化和三者间的相关性,进一步揭示三者联合检测对AMI诊断的临床意义。结果表明,AMI组血浆BNP、MYO、cTnI的检测结果与健康对照组相比较均有明显差异(P<0.05),AMI两组中KI组与KⅡ-Ⅳ组比较均有明显差异(P<0.05);AMI患者的血浆BNP与MYO及cTnI之间具有较好的正相关;AMI组BNP、MYO、cTnI单项及三项联合测定的敏感性分别为93.6%、80.8%、91.5%、97.8%;特异性分别为95.2%、64.3%、95.2%、71.4%。因此,血浆BNP可作为临床预测冠心病患者发生AMI危险性的指标;BNP、MYO及cTnI联合检测可提高AMI早期诊断的敏感性。 To investigate the early diagnostic value of combined detection of plasma brain natriuretic peptide(BNP),myoglobin(MYO) and cardiac tropnin I(cTnI) in patients with acute myocardial infarction.The concentrations of BNP,MYO and cTnI in 47 patients with acute chest pain were determined by chemical immune fluorescence.The results showed that the levels of BNP,MYO and cTnI in patients with AMI were higher than that of control group(P〈0.05);and the BNP,MYO and cTnI levels in KⅡ-Ⅳ were higher than that of KI group(P〈0.05).The significant positive correlation was found between plasma BNP,MYO and cTnI in patients with AMI.The sensitivity of BNP,MYO,cTnI detection and their combined detection in AMI diagnosis was 93.6%,80.8%,91.5% and 97.8% respectively,and the specificity in AMI diagnosis was 95.2%,64.3%,95.2% and 71.4%respectively.The results indicate that the BNP may serve as a predictor of AMI.The combined detection of BNP,MYO and cTnI could improve the detection sensitivity of AMI in early diagnosis.
