糖尿病肾病中医证型及其相关因素的多元分析

Multivariate analysis of Chinese medicine syndrome type and its correlated factors of diabetic nephropathy

目的探讨糖尿病肾病(DN)患者的中医证型的分布特点及其与实验室检查的关系,为今后中西医结合诊治本病的客观化提供依据。方法对155例DN患者的人口学、中医证候学及实验室检查的相关资料进行流行病学的统计和分析。结果 155例患者中医证型本证以肝肾阴虚证最多(34.2%),其次为气阴两虚证(21.3%),脾肾阳虚证最少(13.5%),标证中血瘀证出现频次居首(36.1%);本证证型分布与血清肌酐、尿β2微球蛋白、尿免疫球蛋白(尿IgG)水平均具有相关性。结论患者肾衰竭的程度与DN的中医证型具有相关性,可作为中医辨证客观化指标之一。

Objective It is to explore the distribution characteristic of syndrome type and its relationship with experimental examination in patients with diabetic nephropathy（DN） , and to provide objective evidences for the treatment with traditional Chinese and western medicine for this disease. Methods Epidemiology statistics and analysis of correlated data including de- mography, Chinese medical symptomatology and experimental examination of 155 patients with DN were performed on. Results The basic syndrome of Chinese syndromes that was the most was Liver-kidney Yin deficiency （34.2%） , Qi-Yin deficiency was less （21.3%） , and Sleep-kidney Yang deficiency was the least（ 13.5% ） in the patients. Blood stasis type which was 36.1% was the most in appearance syndromes. The distribution of basic syndromes was related with the levels of serum creatinine, urine β microglobulin, urine IgG. Conclusion The failure degrees of kidney function is correlated with syndrome type of DN, and it can be used as one of the objective indexes of traditional Chinese medical differentiating zheng.
