尿足细胞标志蛋白、血清胱抑素和高敏C反应蛋白联合检测诊断早期糖尿病肾病的价值 被引量:10

摘要 目的探讨糖尿病患者尿足细胞标志蛋白(PCX)、血清胱抑素C(CysC)和高敏C反应蛋白(hs-CRP)的变化对早期糖尿病肾病的诊断价值。方法对糖尿病患者(110例)和正常人(45例)尿PCX、血清CysC、hs-CRP和肌酐(Cr)水平进行联合检测,观察上述指标与病程和病情的关系。结果糖尿病患者尿PCX、血清CysC和hs-CRP水平显著高于正常人,而血Cr水平与正常人比较无显著性差异。随着糖尿病病程的延长和肾脏病变的加重,上述各项指标增加更为明显。结论联合检测糖尿病患者尿PCX、血清CysC和hs-CRP有助于糖尿病肾病的早期诊断。 Objective It is to explore the clinical diagnostic value of eombinative measuring of urinary podoealyxin (PCX) , serum cystatin C(CysC) and high sensitive C reactive protein( hs- CRP) for early diabetic nephropathy in patients with diabetes mellitus(DM). Methods The levels of urinary PCX, serum CysC, hs -CRP and ereatinine (Cr) in 110 patients with DM and 45 healthy control subjects were determined. The relation of the makers mentioned above and the course of disease or pathogenetic condition was studied. Results The levels of urinary PCX, serum CysC and hs - CRP in patients with DM were significantly higher than those in healthy control( P 〈 0. O1 ) ,while the levels of serum Cr had no significant difference between the two groups. Among the diabetic patients, the changes of the levels of urinary PCX, serum CysC and hs - CRP were more obvious in those with prolonged course of disease or advanced impairment of renal funetion(P 〈 0.01 or 〈0.05). Conclusion Combined determination of urinary PCX, serum CysC and hs - CRP is helpful for diagnosis and assessment of early diabetic nephropathy.
