摘要
目的 探讨利用银杏叶注射液辅助治疗慢性阻塞性肺疾病急性加重期(AECOPD)患者的外周血D-二聚体(D-dimer,DD)和抗凝血酶Ⅲ(ATⅢ)变化及其临床意义。方法分别于入院时、治疗后2周抽取42例AECOPD患者静脉血并测定其血浆DD和ATⅢ水平,与健康对照组比较,并将AECOPD分为传统治疗组(A组)和银杏叶治疗组(B组),比较两组治疗后2周DD和ATⅢ水平的变化。结果42例AECOPD患者入院时DD(584.4±193.8)μg/L高于32名对照组(190.3±101.9)μg/L,入院时ATⅢ(76.29±11.72)%低于对照组(90.70±7.05)%,差异有统计学意义(P〈0.01);两组治疗前DD水平和ATⅢ差异无统计学意义(P〉0.05);传统治疗组21例治疗2周DD降至(421.9±156.2)μg/L,ATⅢ升高至(83.10±8.32)Voo;银杏叶治疗组21例降至(302.5±166.0)μg/L,ATⅢ升高至(88.79±9.32)%;治疗后DD水平均较治疗前有所下降,ATIⅢ水平均较治疗前有所升高,差异均有统计学意义(P值均〈0.05);银杏叶治疗组在治疗后的DD水平比传统治疗组明显减低,银杏叶治疗组在治疗后ATⅢ水平比传统治疗组明显升高,差异有统计学意义(P值均〈0.05)。结论银杏叶注射液能明显降低DD,显著增加ATⅢ,对AECOPD有一定的治疗价值,与AECOPD的预后有关。
Objective To investigate the changes and clinic significance of plasma D-dimer (DD) and antithrombin (ATⅢ) levels in Yinxingye injection in patients with acute exacerbations of chronic obstructive pulmonary disease (AECOPD). Methods The plasma levels of DD and ATⅢ were detected before and after 2 weeks of treatment in 42 patients with AECOPD and 32 healthy persons as control subjects. The patients were divided into group of ordinary therapy (group A) and group of Yinxingye injection (group B). The plasma levels of DD and AT Ⅲ were compared before and after treatment between the groups. Results Before treatment, the level of DD (584.4±193.8) /,g/L was significantly higher in 42 patients of AECOPD than in control subjects (190.3±101.9) /,g/L, the level of AT lll (76.29± 11.72) ~ was significantly lower than that in control subjects (90.70±7.05) % there was significant statistical difference between the two groups of AECOPD and control subjects (P〈 0.01). There was no significant statistical difference the levels of DD and ATⅢ between group A and group B ( P〈0.05). In group A, the levels of DD decreased to (421.9± 156.2) μg/L and ATⅢ increased to (83. 10±8. 32) % after 2 weeks of therapy ( P〈0.05). In group B, DD decreased to (302.5±166.0) μg/L and AT HI increased to (88.79± 9.32) % after 2 weeks of therapy ( P 〈0.05). There was significant statistical difference in the lower DD and the higher ATⅢ between before treatment and after therapy ( P 〈0.05). After therapy ,the level of DD in group B was significantly lower than that of group A, and the level of ATⅢ in group B was significantly higher than that of group A. There was significant statistical difference between group B and group A after therapy ( P 〈0.05). Conclusions There was significant lower in the level of DD and significant higher in the level of AT Ⅲ after the therapy of Yinxingye injection in patients with AECOPD ,which were also related to the outcome of AECOPD.
