我院2007-2010年鲍曼不动杆菌感染的耐药性分析 被引量:18

Analysis of Antimicrobial Resistance of Acinetobacter baumannii Infection in Our Hospital from 2007 to 2010

摘要 目的:分析临床分离的鲍曼不动杆菌的分布特点及其耐药情况表型,为临床合理应用抗菌药物提供依据。方法:采用全自动微生物分析仪Microscan WalkAway 40对我院2007-2010年临床分离的鲍曼不动杆菌进行菌种鉴定和最低抑菌浓度(MIC)测定,所有数据应用WHONET5.4软件进行统计学分析。结果:分离的鲍曼不动杆菌共174株,主要来源于痰标本(149株,占85.6%);病房主要集中于重症监护病房(61株,占35.1%),其次是呼吸内科(46株,占26.4%);患者多见于60岁以上老年患者(83株,占47.7%)。其对14种不同抗菌药物的耐药率总体呈逐年上升趋势,特别是对亚胺培南耐药率逐年上升,从17.6%上升到64.5%;还发现88株(50.6%)泛耐药菌株,并且检出率不断增加。结论:鲍曼不动杆菌对各种抗菌药物的耐药性逐渐增强,多重耐药和泛耐药鲍曼不动杆菌分离率逐年增高;我们应警惕并高度重视该菌感染及耐药监测,以减少耐药菌株的产生和播散。 OBJECTIVE: To analyze the distribution characteristics and antimicrobial resistance of isolated Acinetobacter baumannii, and to provide reference for rational use of antibiotics in the Clinic. METHODS: Microscan WalkAway 40 automatic system (America) was used to identify the species and the minimal inhibitory concentration (MIC) for.4, baumannii which were separated from 2007 to 2010. All the data were analyzed by WHONET 5.4 software. RESULTS:A total of 174 strains of A. baumannii were isolated, which mainly came from the sputum samples of patients( 149 strains, 85.6% ). In the distribution of hospital departments, ICU was the most (61 strains, 35.1% ), respiratory department was the second (46 strains, 26.4% ). 47.7% were older suffers aged more than 60 years (83 strains)..4, baumannii was resistant to 14 kinds of antibiotics increasingly, especially to imipenem, from 17.6% to 64.5%; and we found 88 strains were multidrug-resistant strains, and the relevance ratio was increasing sharply. CONCLUSIONS: Antimicrobial resistance of A. baumannii and the isolation rate of multidrug-resistant and pandrug-resistant A. baumannii are increasing year by year. So we must keep an eye on A. baumannii infection and its drug-resistance changes in order to prevent the emergence and spread of drug-resistant strains.
