摘要
目的初步探讨兔VX2肝癌种植瘤灶多层螺旋CT(MSCT)增强表现与微血管密度(MVD)的相关性。方法对42只新西兰大白兔建立VX2肝癌模型,成功建立模型32只,于成功建模后第14、21、28天行全肝MSCT平扫及增强后动脉期、门脉期扫描检查,观察肿瘤生长情况,分析瘤灶边缘部与中心部不同阶段不同期相的CT强化特征及幅度变化,同时利用免疫组化SP法,用CD34标记兔VX2肝癌种植瘤灶微血管,进行免疫组化染色,分别计算瘤灶边缘部、中心部瘤组织的MVD值,并将不同时期兔VX2肝癌种植瘤灶的CT强化特征及幅度与MVD值进行相关性分析,以探讨二者的相关性。结果 (1)兔VX2肝癌种植瘤灶边缘部增强后动脉期及门脉期CT值的变化具有统计学差异(P<0.05);瘤灶中心部平扫、动脉期、门脉期CT值的变化具有统计学差异(P<0.05);(2)瘤灶边缘部及中心部14、212、8 d MVD值的间的差异具有统计学意义(P<0.05);(3)瘤灶边缘部21、28 d MVD值的变化与动脉期CT值的变化具有正相关性(r分别为0.59、0.72,P<0.05),瘤灶中心第21天MVD值的变化与动脉期CT值的变化具有正相关性(r=0.61,P<0.05);(4)瘤灶边缘部3个时间点MVD值的变化与瘤灶直径均有相关性(r分别为0.59、0.78、0.69,P<0.05)。结论 MSCT增强扫描后的图像特征和强化幅度能够在一定程度反映瘤灶的微循环状态,为评价肿瘤的生物学特性提供了一种无创简便的检查方法。
Objective To evaluate the correlation between enhancement features of Multi-Slice Sprial CT(MSCT) and the Microvessel Density(MVD) in the transplanted model of VX2 liver carcinoma in rabbit.Methods Forty two New Zealand rabbits and 32 rabbits were established with VX2 liver carcinoma successfully.CT normal scan and enhanced scan were taken to the 32 rabbits at 14 d,21 d and 28 d after haven established successfully.The imaging of plain,arterial phase and portal phase were obtained,the enhancement characteristic of the tumor center and border at different stage were analyzed.And the immunohistochemical analysis were carried out to examine the quantity of MVD by using antibodies CD34 in tumor center and border at the same time.Then correlation analysis was performed according to the different enhancement characteristic and MVD at different stages.Results(1) The CT value change of the edge of rabbit VX2 tumor at arterial phase and portal phase showed significant differences(P〈0.05);the CT value change about the center of rabbit VX2 tumor at plain scan,arterial phase and portal phase showed a significant differences(P〈0.05);(2) The MVD of VX2 tumor center and border at 14 d,21 d and 28 d after haven established successfully with significant differences(P〈0.05);(3) Positive relations were shown between the variation of the CT value at arterial phase and MVD of the tumor border at 21d and 28 d(r=0.059,r=0.072,P〈0.05),positive relations were shown between the variation of the CT value at arterial phase and MVD of the tumor center at 21d(r=0.061,r=0.072,P〈0.05);(4) Positive relations were shown between the MVD of tumor border and tumor size at 14 d,21 d and 28 d,r=0.059(r=0.078,r=0.069,P〈0.05).Conclusion The imaging character and enhancement degree of MSCT enhanced scanning can reflect the MVD of the tumor,MSCT enhanced scanning is a non-invasive and simple method to evaluate the biological characteristics of tumor.
