兔VX2肝癌模型的建立及其CT灌注评价

摘要 目的探讨建立兔VX2肝癌模型的关键技术及兔VX2肝癌CT灌注成像的血流动力学特点。方法 35只新西兰大白兔随机挑选2只,于后肢皮下注射肿瘤细胞保留瘤种(荷瘤组),余33只随机分为实验组25只和对照组8只,实验组再随机分为实验1组10只,实验2组15只。对照组开腹种植明胶海绵;实验1组、2组分别采用CT引导下针头注入法及包埋法接种肿瘤细胞于兔肝左叶。实验组接种10 d后行CT扫描判断接种是否成功,并比较两种接种方法的差异。成功者3周后行CT灌注扫描,应用CT灌注软件程序计算肝脏的灌注参数,包括:肝动脉灌注指数(Hepatic Arterial Fracture,HAF)、门静脉灌注指数(Hepatic Portal Perfusion Index,HPPI)、血流速(Blood Flow,BF)、血容量(Blood Volume,BV)、平均通过时间(Mean Transit Time,MTT)、表面通透性(Per-meability Surface,PS)和到达时间(Time of Arrival,TO)。对照组同时行此项检查,比较实验组及对照组各灌注参数的异同。结果实验1组肿瘤种植成功率60%,实验2组种植成功率100%;种瘤3周后实验组与对照组相比,肿瘤区域CT灌注参数:实验组HAF值、PS值、BF值分别为:(0.90±0.03)、(42.73±8.47)ml.100 g-1.min-1、(225.93±5.64)ml.100 g-1.min-1;对照组HAF值、PS值、BF值分别为:(0.26±0.03)、(23.69±4.40)ml.100 g-1.min-1、(163.42±4.01)ml.100 g-1.min-1;HAF值、PS值、BF值两组间差异均具有统计学意义(P<0.05)。实验组TO值和HPPI值分别为:(2.11±0.54)ml/100 g(、0.11±0.27)ml.100 g-1.min-1;对照组TO值和HPPI值分别为:(9.63±1.34)ml/100 g、(0.77±0.08)ml.100 g-1.min-1,实验组TO值和HPPI值均较对照组减低,差异具有统计学意义(P<0.05);实验组与对照组MTT、BV的差异无统计学意义(P>0.05)。结论兔VX2肝癌模型的建立,以包埋法较针头注入法成功率更高;CT灌注成像能反映瘤体的灌注特点,并能评价其血供及血流动力改变特点。 Objective To explore the way that building VX2 liver tumor model of rabbits and CT perfusion imaging of the hemodynamic feature of hepatic VX2 tumor in rabbits.Methods Thirty five New Zealand white rabbits were enrolled,and divided into 4 groups stochastically,2 were as the Grafted Rabbits,the grafted VX2 carcinoma was maintained through serial transplantation into the hind limb of the 2 rabbits by subcutaneous injection;10 were designated in the first study group;15 were designated in the second study group,and the other 8 were designated in the control group.In the first study group the rabbits were injected with VX2 tumor in the left lobe of the liver under CT guidance while in the second study group,the rabbits were embed with VX2 tumor,and the 8 rabbits of the control group were embed with gelatin sponge.CT plain and enhanced scan was performed 10 days later and liver CT perfusion imaging scan was performed 3 weeks later after the operation,then measured the BF(blood flow),BV(blood volume),PS(permeability surface),HAF(hepatic arterial fracture),MTT(mean transit time),TO(time of arrival)and HPPI(hepatic portal perfusion index) using the functional CT perfusion software program,according to this CT perfusion parameters the feature of the hemodynamic was evaluated.The 8 rabbits in the control group were performed in the same way.Results The successful rates of implantation were 60% in the first study group and 100% in the second study group;Compared with the control group,HAF,PS,BF in study group increased significantly,HAF:(0.90±0.03) vs(0.26±0.03)(P〈0.05),PS:(42.73±8.47) ml·g^-1·min^-1vs(23.69±4.40) ml·g^-1·min^-1P〈0.05),BF:(225.93±5.64) ml·g^-1·min^-1vs(163.42±4.01) ml·g^-1·min^-1P〈0.05);TO and HPPI decreased significantly: TO(2.11±0.54) ml/100 g vs(9.63 ±1.34) ml/100 g(P〈0.05),HPPI:(0.11±0.27) ml·g^-1·min^-1s(0.77 ±0.08) ml·g^-1·min^-1(P〈0.05);MTT and BV did not differ between the two groups.Conclusion The rabbits model bearing VX2 liver tumor is suitable for interventional experimental study,the embedded method is superior to the injection method;CT perfusion imaging may reflect the feature of tumor perfusion and evaluate the blood supply and the hemodynamic chang of hepatic VX2 tumor in rabbits.
