β_2微球蛋白与乳酸脱氢酶对非霍奇金淋巴瘤治疗效果的判断价值 被引量:8

摘要 目的探讨血清β2微球蛋白(β2-MG)和乳酸脱氢酶(LDH)在判断非霍奇金淋巴瘤(NHL)有无骨髓浸润及治疗效果中的意义。方法回顾性分析106例经病理确诊的NHL初治患者,按骨髓涂片检查结果分为淋巴瘤细胞白血病组27例(淋瘤白组),有骨髓浸润组26例(浸润组),无骨髓浸润组53例(无浸润组)。分别于化疗前后检测血清β2-MG和LDH水平,比较3组间有无差异。结果 3组患者β2-MG水平比较差异有统计学意义(P<0.01),其中浸润组和淋瘤白组β2-MG水平均高于无浸润组(P<0.01);淋瘤白组β2-MG水平高于浸润组(P<0.01)。3组患者LDH水平比较差异有统计学意义(P<0.01),其中浸润组和淋瘤白组LDH水平均高于非浸润组(P<0.01)。3组患者应用化疗药物治疗后β2-MG和LDH水平均有所下降,差异有统计学意义(P<0.05)。结论β2-MG和LDH可作为NHL患者是否存在骨髓浸润及其治疗效果的临床判断指标。 Objective To discuss the importance of scram β2 - MG ( beta 2 microglobulin) and LDH ( lactate dehy- drogenase) in deciding whether there is bone marrow infiltration or not in non - Hedgkin's lymphoma (NHL) and to discuss their significance in treatment. Methods 106 patients were chosen as research objects. They are all first - visit patients. By bone marrow aspiration, 27 patients were diagnosed as lymphoma cell leukemia ( lymphoma cell leukemia group), 26 patients as NHL with bone marrow infiltration (infiltration group) and 53 patients without bone marrow infiltration (no infiltration group) . The level of serum β2 - MG and LDH were detected and compared before and after treatment. Results There was statistical difference among the 3 groups in β2 -MG levels (P 〈0. 01 ) . Infiltration group and lymphoma cell leukemia group. The levels of β2 -MG in infiltration group and lymphoma cell leukemia group were higher than that in no infiltration group ( P 〈 0. 01 ) . The level of β2 - MG in lymphoma cell leukemia group was higher than that in infiltration group (P 〈0. 01 ) . There was statistical difference a- mong the 3 groups in LDH levels. The levels of LDH in infiltration group and lymphoma cell leukemia group were higher than that in no infdtration group (P 〈0. 01 ) . The level of LDH in lympbema cell leukemia group was higher than that in infiltration group ( P 〈 0. 01 ) . The levels of β2 - MG and LDH among the 3 groups decreased after chemotherapy drags treatment ( P 〈 0. 05 ). Condusion β2 - MG and LDH can be used as indicators in judging whether there is bone marrow infdtration or not and they are important clinical therapeutic markers of NHL.
