腹盆腔CT扫描在评估卵巢癌分期及肿瘤细胞减灭术可行性中的应用价值

Preoperative Abdominpelvic CT Prediction of Staging and Optimal Cytoreduction in Ovarian Carcinoma

目的 探讨术前腹盆腔ＣＴ 扫描在评估卵巢癌分期及肿瘤细胞减灭术可行性中的应用价值。方法 ４０例卵巢癌术前一周内行腹盆腔ＣＴ扫描，术后将其影像表现与手术病理对照分析。结果 ４０ 例卵巢癌ＣＴ 分期准确性７５ ％ ，Ⅲ期分期的准确性８５％ ，ＣＴ 对横膈、肝包膜，子宫直肠窝转移病变检测的敏感性为１００ ％ 、６９ ％ ，网膜转移敏感性８８％ 。ＣＴ评估成功的肿瘤细胞减灭术敏感性９１％ ，不可切除病变敏感性为６６ ％ 。

Objective To evaluate ovarian cancer staging and tumor resectalibity with computed tomography. Methods Forty women underwent CT,lmaging was performed 1 week of sugical staging. Radiologic.surgical and histopathologic findings were compared. Results Over all staging accuracy with computed Tomography imaging was 75%,accuracy was highest in patients with stage Ⅲ disease 85% for CT. The sensitivity of detection on liver surface、diaphragmatic and in the cul desac impants with CT was 100%、69%. The sensititity of omental involvement with CT was 88%. The sensitivity of 91% for prediction of optimal cytoveduction,nonresectability was 66%. Conclusion Preoperative abdominopelvic computed tomogrphic is should be used as excellent method of staging in ovarian cance and prediction of tumor resectability. [
