摘要
目的探讨气阴两虚证型患者凋亡相关基因的表达,观察善胃Ⅲ号治疗气阴两虚型胃癌前病变的疗效,并从细胞凋亡和相关基因的表达方面探讨其作用机理。方法 77例患者均经胃镜与病理检查证实为胃癌前病变,随机分成两组,光镜观察治疗前后胃黏膜的增生情况,并采用Tunel法检测细胞凋亡,免疫组化检测fas、fas-L、bel-2、bax的表达。结果治疗组(善胃Ⅲ号,气阴两虚型)有效率79.31%(29例),优于对照组(猴头菌片)29.17%(48例)。治疗组细胞凋亡率和fas-L、bax表达率分别从(15.46±11.91)%、46.67%、42.88%升高至(71.43±19.87)%、76.92%、68..42%;而bcl-2表达率从87.50%下降至20.00%。对照组猴头菌片对上述指标无明显作用。结论善胃Ⅲ号能有效治疗气阴两虚型胃癌前病变,明显诱导胃癌前病变胃黏膜细胞凋亡,增强fas-L、bax表达而抑制bcl-2表达。
Objective To explore expression of apoptosis - related genes in the pattern of Qi - yin deticiency symptom. To study the effect of Benefiting Stomach Ⅲ on treating precancerosis of gastric cancer, and the mechanism of apoptosis and its relative genes expression. Methods 77 cases of precancerous lesions of gastric cancer were divided into 2 groups at random, Chinese herbal observation group ( treatment group blow, the pattern of Qi - yin deficiency symptom) and Herieium tablet on market sell in control group ( control group blow). The hyperplasia of gastric epithelial cell was observed by microscope. The apoptosis and expression of fas, fas - L, bel - 2, and bax were detected by Tunel and method separately. Results The efficiency rate of treatment group ( 79.31% , n = 29 ) was higher than that of control group (29. 17% ,n = 48 ). Apoptosis, fas - L and bax expression rate were significantly increased from ( 15.46 ± 11.91 ) %, 46.67% ,42. 88% to (71.43 ± 19.87) % ,76.92% ,68.42%, but bcl - 2 expression rate was decreased from 87.50 % to 20. 00% in treatment group. Control group didn't display obvious effect on genes expression. Condusion Genesis of the pattern of Qi - yin deficiency symptom may be related with the apoptosis - related genes. Benefiting Stomach Ⅲ is effective to precancerosis of gastric cancer (the pattern of Qi -yin deficiency symptom), induce significantly gastric epithelial apoptosis, increase fas - L,bax expression but suppress bcl - 2 expression.
