速度向量成像技术对原发性高血压血瘀证患者颈动脉弹性的初步评价

摘要 目的:应用速度向量成像(VVI)技术初步分析原发性高血压血瘀证患者颈动脉运动特点,探讨VVI技术检测颈动脉壁弹性的临床应用价值。方法:73例研究对象根据标准分为原发性高血压非血瘀证组(非血瘀组,n=22)、原发性高血压血瘀证组(血瘀组,n=26),对照组(n=25);应用VVI技术检测颈总动脉的正向峰值应变(S+max)、负向峰值应变(S-max),正向峰值应变率(SR+max)及负向峰值应变率(SR-max)。结果:①S-max和S+max组间比较血瘀组颈动脉S-max和S+max均显著小于对照组(p<0.05),非血瘀组颈动脉S-max和S+max与对照组比较无显著性差异(p>0.05);S-max和S+max组内比较三组组内颈动脉各壁S-max和S+max均无显著性差异(p>0.05)。②SR-max和SR+max组间比较血瘀组、非血瘀组颈动脉SR-max显著小于对照组(p<0.05),血瘀组颈动脉SR-max显著小于非血瘀组(p<0.05);血瘀组颈动脉SR+max高度显著小于对照组(p<0.01);非血瘀组颈动脉SR+max显著小于对照组(p<0.05);SR-max和SR+max组内比较三组组内颈动脉各壁SR-max和SR+max均无显著性差异(p>0.05)。结论:VVI技术可较好地判断颈总动脉管壁弹性改变,为临床早期诊断动脉硬化提供依据。 Objective:To study the velocity,strain and strain rate of the carotid arterial wall by using velocity vector imaging(VVI) in blood stasis patterns of essential hypertensive patients and to investigate the clinical value of evaluating carotid arterial wall motion with VVI.Methods:73 patients were assigned to three groups,22 patients were non-blood stasis syndrome(the non-blood stasis group) and 26 patients were blood stasis syndrome(the blood stasis group) and the control group contained 25 normal people.The carotid arterial wall motion in the blood stasis group,the non-blood stasis group and the control group were examined with VVI.The nagetive maximum strain(S-max) and forward maximum strain(S+max) and nagetive maximum strain rate(SR-max) and forward maximum strain rate(SR+max) of the common carotid arterial wall were measured.Results:The S-max and S+max in the blood stasis group was lower than the control group(p0.05),The S-max and S+max had no significant difference among the non-blood stasis group and the control group(p0.05).The S-max and S+max had no significant difference in every carotid arterial walls in the blood stasis group and the non-blood stasis group and the control group(p0.05).The SR-max in the blood stasis group and the non-blood stasis group was lower than the control group(p0.05).The SR+max in the blood stasis group was utmostly lower than the control group(p0.01).The SR-max in the non-blood stasis group were lower than the control group(p0.05).The SR-max and SR+max had no significant difference in every carotid arterial walls in the blood stasis group and the non-blood stasis group and the control group(p0.05)Conclusion :VVI is an effective technique for evaluating the flexibility of the common carotid arterial wall.
