目的 推荐腋窝哨兵淋巴结定位活检术(SLND)。方法 1.染色法——将染料注入乳房包块及周围组织或注入活检腔内,通过兰染的淋巴管找到哨兵淋巴结(SIN)。常用染料:①淋巴兰;②Lsosnlphan兰;③美兰;④荧光性染料cyalnme;⑤专利兰-Ⅴ。2。同位素示踪-药物及途径:①^(99m)TC-M1B1:由健侧正中静脉快速注入;②^(99m)TC胶体硫:注入肿瘤周围或活检腔内或对未扪及包块者在影像学及立体定位的引导下注入局部;③^(99m)TC标记的人体白蛋白:术前一天于肿瘤表面皮内注射;④FDC-PET技术。结果 联合应用SLND加法检出率达100％,无假阴性,预测准确率100％。结论 早期乳腺癌SLND阴性时可免除ALND,使患者可获得很多好处。

Objective To recommend the armpit sentry lymphaden positioned living inspection (SLND). Methods 1. Dyeing-inject the dye into the breast mass and the around organs or into the living inspection chamber, to find the sentry lymphaden (SLN) through bluedyed lymphatics. Common dyestuff:①Lymph blue. ②Isosnlphan blue.③ Melan. ④Fluorescent cyalnme. ⑤Patent blue-V. 2. Isotope indicated track-medicine and way. ①99m'TC-M1 B1:quick injection into the middle vein at healthy side;②99mTC colloid sulphur: injection into the near tumor or the living inspection chamber or partial injection to the patient with untouched tumor under guidance of reflex image and stereoscopic positioning;③99mTC labelled human white protein: One day pre-operation skin injection onto the tumor surface;④FDG-PET Tech.Results Joined applicable SLND methods to reach detection ratio of 100%, without faulty Negative, forecast correctness 100% . Conclusion For early stage breast cancer SLND Negative, ALND can be eliminated,which enables patients benefit a lot.
