380例发热患者的骨髓像分析

Analysis of bone marrow image of 380 febrile patients

目的:探讨骨髓形态学检查对长期不明原因发热待查(FUO)患者查找病因的临床意义。方法:回顾性总结分析2007年～2010年瑞金医院血液科骨髓细胞室符合FUO诊断标准的380例患者资料。结果:380例患者中骨髓大致正常的159例(41.8%),粒细胞减少症39例(10.3%),恶性肿瘤24例(6.3%),淋巴瘤22例(5.8%),MDS 13例(3.4%),骨髓稀释21例(5.5%),骨髓坏死13例(3.4%),其余7例(0.8%)。结论:骨髓检查不仅是血液系统疾病确诊的方法之一,还能对多种发热疾病提供线索和依据,是FUO的重要检查手段之一,对临床有重要的辅助诊断价值。

Objective To explore clinical significance of the bone marrow morphology in long-term unexplained fever inestigate（FUO） patients.Methods Retrospective analysis of 380 patients data in ruijin hospital bone marrow cells room from 2006～2009 that met diagnostic criteria of FUO.Results 159 out of 380 cases had an approximately normal bone marrow image（41.8%）,39 cases of granulocytopenia（10.3%） 24 cases of malignant tumor（6.3%）,22 cases of lymphoma（5.8%）,13 cases of MDS（3.4%）,21 cases of bone marrow dilution（5.5 %）,13 cases of bone marrow necrosis（3.4%）,13 cases of others（0.8%）.Conclusion Bone marrow test is not only one of the hematological system disorder diagnostic method,but also provides clues to various fever disease,thereby perform an important role in FUO auxiliary diagnostic method.
