股骨骨折患者早期康复护理依从性影响因素分析

Multi-factor Analysis of Compliance of Patients with Femoral Fracture in Early Rehabilitation Nursing

目的分析股骨骨折患者早期康复护理依从性的影响因素,探究深层原因,以提高股骨骨折患者早期康复护理效果,改善患者术后远期生存质量。方法选择符合纳入标准的于2010年3—9月入住骨科的股骨骨折患者以及陪护人员,通过自制调查问卷(分患者版和陪护人员版)分别对患者和陪护人员进行调查。患者版问卷包括患者年龄、性别、文化程度、婚姻状况、家庭月收入、付费方式等基本信息,以及康复期望值、疼痛评分、手术方式、患者焦虑状态等。陪护人员版问卷包括陪护人员年龄、文化程度、与患者关系等基本信息,以及陪护人员对疾病认识、对医护人员工作认可度等多个方面。同时,以"医护一体"工作模式就医护人员查房对患者的康复训练医嘱、护嘱、健康教育内容等进行记录,并评价患者早期康复护理措施执行情况。结果股骨骨折患者早期康复训练依从性与患者的文化程度、家庭月收入、就医期望值、疼痛评分和陪护人员对疾病的认识以及对医护人员临床工作认可度等因素有关;与患者焦虑状况没有明显的相关性。结论在临床医疗护理过程中,股骨骨折患者早期康复护理依从性受多种因素影响,应根据患者个体差异性制定独特的早期康复护理计划,同时,医护人员在重视患者治疗与健康教育过程中,必须同时重视对患者陪护人员的指导,提高其对疾病的认识,建立良好的护患关系促进患者早期康复。

Objective To explore factors exerting influence on compliance of patients with femoral fracture in early rehabilitation nursing and to improve nursing quality and quality of life of patients. Methods A self-designed questionnaire was employed among patients and their caregivers for general information and information such as pain score, anxiety, disease cognition and acceptability of work of medical staff. Meanwhile, medical order and health education of patients were recorded as well as the evaluation of nursing measures in early rehabilitation. Results Factors that had impact on patients＇ compliance included patients＇ educational background, family income, expectation to doctors, pain score, disease cognition of caregivers, caregivers＇ acceptability of medical staff＇s work while there was no significant relationship between the compliance and patients＇ anxiety. Conclusion Specific nursing strategies of early rehabilitation for patients should be established and guidance for caregivers should be provided as well.
