胸水ADA、TB-DNA和IFN-γ的联合检测对结核性胸膜炎的诊断价值评估 被引量:10

摘要 目的评价胸水中腺苷脱氨酶(ADA)、结核分枝杆菌脱氧核苷核酸(TB-DNA)和干扰素γ(IFN-γ)的联合检测在结核性胸膜炎的临床诊断意义。方法对172例疑似结核性胸膜炎患者的胸水标本进行ADA、TB-DNA和IFN-γ检测。结果 172例患者的胸水中,71例确诊为结核性胸膜炎,TB-DNA的灵敏度、特异度为40.85%、99.01%;ADA为61.97%、74.26%;IFN-γ为77.5%、95.2%。TB-DNA或IFN-γ组合的灵敏度和特异度最好,具有很好的诊断价值。ADA和IFN-γ组合的灵敏度最高,与其他2种方法有显著性差异。3种方法联合检测的敏感性和准确性高于2种方法联合检测,具有较高的临床应用价值。结论胸水ADA、TB-DNA、IFN-γ3种方法联合应用,是一种快速、灵敏度和特异度很高的诊断结核性胸膜炎的方法。
