不同刺激量捻转手法针刺后三里穴对高血压家兔血压的影响 被引量:17

Effect of Acupuncture with Different Lifting-thrusting and Twirling Intensities at Point Housanli on Blood Pressure in Hypertension Rabbits

摘要 目的观察不同频率刺激参数的捻转手法对实验性高血压家兔血压的实时影响,探讨针刺手法的量效关系。方法将50只新西兰家兔随机平均分为空白组、模型组、治疗组(大、中、小刺激量组)5组,应用在体捻转手法仪在治疗组家兔后三里穴行大、中、小刺激量捻转手法,采用颈总动脉插管测量法全程记录家兔血压值变化并观测与分析。结果捻针1.5~2 min时,中刺激组出现降收缩压效应(P<0.05),小刺激组出现降舒张压效应(P<0.05)。中刺激组在留针20 min内均有降收缩压和舒张压效应(P<0.05);小刺激组在0~10 min和15~20 min时有降收缩压效应,在0~10 min时有降舒张压效应(P<0.05);中刺激组在10~20 min时的降收缩压效应和在5~10 min时的降舒张压效应均优于大刺激组(P<0.05)。起针后,仅中刺激组在20 min内有降收缩压效应(P<0.05)。大刺激组在实验全过程无明显降收缩压和舒张压的效应(P>0.05)。结论 不同刺激量的捻转手法对高血压家兔的血压影响存有不同程度的效应差异。中、小刺激量有降压效应,而大刺激量则无。其中,中刺激量降收缩压疗效显著,小刺激量则降舒张压疗效较明显。 Objective To investigate the real time effect of different frequencies of lifting-thrusting and twirling manipulation (stimulation parameters) on blood pressure in experimental hypertension rabbits and explore the intensity-effect relationship of acupuncture manipulation. Methods Fifty New Zealand rabbits were randomly allocated to five groups: blank, model and treatment (high, moderate and low stimulation intensity). Point Housanli (ST 36) was acupunctured with high, moderate and low stimulation intensities of lifting-thrusting and twirling manipulation using an in vivo twirling manipulator in the treatment groups of rabbits. Rabbit blood pressure was recorded by common carotid artery catheterization throughout the experiment. The changes were observed and analyzed. Results At 1.5-2 min after twirling the needle, the effect of reducing systolic pressure occurred in the moderate stimulation group (P〈0.05) and the effect of reducing diastolic pressure occurred in the weak stimulation group (P〈 0.05). The effects of reducing both systolic and diastolic pressures occurred in the moderate stimulation group during 20 min retention of needle (P〈0.05). In the weak stimulation group, the effect of reducing systolic pressure occurred during 0-10 min and 15-20 min after the beginning of needle retention and the effect of reducing diastolic pressure, during 0-10 rain (P〈0.05). The effect of reducing systolic pressure during 10-20 min and the effect of reducing diastolic pressure during 5-10 min were better in the moderate stimulation group than in the weak stimulation group (P〈0.05). After needle withdrawal, only the effect of reducing systolic pressure occurred in the moderate stimulation group during 20 min (P〈0.05). No marked effects of reducing systolic and diastolic pressures occurred in the strong stimulation group throughout the experiment (P〉0.05). Conclusions Different stimulation intensities of twirling manipulation have different degrees of impact on blood pressure in hypertension rabbits. Moderate and low stimulation intensities produce a hypotensive effect but high stimulation intensity does not. Of them, moderate stimulation intensity has the marked therapeutic effect of reducing systolic pressure and low stimulation intensity has the more marked therapeutic effect of reducing diastolic pressure.
