兔脑卒中模型康复实用化及其神经功能缺损评定探讨

摘要 目的:探讨兔脑卒中模型如何适应康复实验研究,并分析康复处理后该模型神经功能缺陷情况。方法:(1)取12只兔,随机分为实验组(8只)和对照组(4只),实验组电凝阻断右侧大脑中动脉,对照组仅暴露右侧大脑中动脉而不予电凝。实验组兔模型为康复进行实用化处理,包括术中防堵脑脊液漏、术后保温、静脉营养支持/灌饲流食、脑卒中急性期神经药物治疗等。(2)根据兔子的行为特征,参考兔Purdy评分和鼠NSS评分标准,对两组兔术前、术后不同时段进行神经功能缺陷评定。结果:(1)单纯手术模型术后2d内死亡2例,其余康复实用化处理后,模型存活3周以上。(2)实验组为康复实用化处理后,术后1周末神经功能缺损评定均分为8±0.3;对照组为3±0.8;术后2周末和3周末两组均分(8±0.6/2±0.7,7±0.4/1±0.8),两组三次比较均有显著差异(P<0.01)。结论:单纯电凝法制备家兔MCAO的脑卒中模型不能满足康复全程实验要求,须对模型为康复进行实用化处理。 Objective:The study in the making of practical model for rehabilitation of stroke and the evaluation of nerve function defect of rabbit.Methods:(1)12 rabbits were randomly divided into the experimental group(8 cases) and the control group(4 cases).The right middle cerebral artery obstruct(MCAO) in the experimental group,only expose in control group.The practical treatment was made for rehabilitation of rabbit model:in operative cerebrospinal fluid leakage was prevented and postoperative for heat preservation,parenteral nutrition support/filling liquid diets of feeding and neurological therapy of stroke.(2)According to the behavior of the rabbit and referencing rabbit Purdy score and rat NSS scoring standards,the scale of rabbits was evaluated in the different periods In preoperative and postoperative.Results:(1)For 2 days 2 cases with surgery alone died.After practical treatment for rehabilitation,the rabbits live above 3 weeks.(2)In the experimental group the neural function defect evaluation are divided into 8±0.3 in a weekend,in the control group are divided into 3±0.8.In two and three the weekend after operation the neural function defect was evaluated again.The averages(8±0.6/2±0.7,7±0.4/1±0.8)in the two groups were significant difference(P0.01).Conclusion:(1)The pure electric method for the study of MCAO stroke model can not meet the requirements.After practical treatment for rehabilitation,the model can guarantee the survival and meet the whole model experiment research needs.
