非穿孔伤玻璃体积血手术疗效及临床观察

Surgical results of vitrous hemorrhage with nonperforating ocular trauma.

目的 探讨非穿孔性眼外伤玻璃体积血和相应的视网膜和脉络膜病变的特征 ,及玻璃体手术疗效。方法 玻璃体切除治疗 91例 ( 96眼 )因碰撞伤、弹崩伤、爆炸伤及Terson综合征等所致玻璃体积血。结果 83眼 ( 86 5 % )获得功能性成功 ,5眼 ( 5 2 % )获得解剖性成功、8眼 ( 8 3 % )失败 ;常见的眼部病变有视网膜、脉络膜破裂、青光眼、牵引性视网膜脱离。结论 外伤性质和积血病程与预后密切相关。弹崩伤、爆炸伤预后较差 。

Objective To evaluate the surgical results and posterior segment with ocular trauma without laceration or rupture.Methods Ninty six eyes with trauma undering posterior segment underwent vitrectomy.Results Of 96 eyes with trauma,83 eyes (86.5%) achieved fanctional success,5 eyee(5.2%)attained anatomic success,8 eyes(8.3%) fautures.Of the 96 eyes,21 eyes were found with tear on retina,17 eye with rupture on choroid,10 eyes with tractive retinal detachment.The 30 eyes achieved final visual acuity of 0.5 or better.Conclusions The prognosis was better in cases with Terson symdrom and without pathy on retina and worse in cases with retinopathy or marked vitreous hemorrhage.
