眼眶非何杰金淋巴瘤的临床表现及病理特征分析

A clinical pathological analysis of orbital non-hodgkin's lymphoma

目的探讨眼眶非何杰金淋巴瘤的临床表现和病理特征。方法选择我院收治的15例眼眶非何杰金淋巴瘤患者,分析其临床特点,并通过组织学观察和免疫组化分析观察其病理学特征。结果①B细胞淋巴瘤为多,T细胞淋巴瘤<1/3,②B淋巴细胞型,临床表现通常为上下睑的内、外侧,眼眶前部或泪腺区可扪及边界清楚,周围不光滑、质地中等无痛性包块;T淋巴细胞型病情发展可以非常急,与眼眶蜂窝织炎难以鉴别;亦可表现为慢性,似炎性假瘤;③眼眶非何杰金淋巴瘤的CT密度和MRI信号表现无特异性;④非何杰金B小淋巴细胞型弥漫性淋巴瘤是眼眶淋巴瘤常见的病理类型。结论对于眼眶非何杰金淋巴瘤来说,根据其临床特点和病理学特征,要尽早病理活检做鉴别诊断,尽量避免误诊而延误最佳治疗时机。

Objective To investigate the clinical pathological features of orbital non-Hodgkin＇s Lymphoma. Methods Clinical pathological analysis and im-munohistochemical staining were employed for 15 cases with Orbital non-Hodgkin＇s Lympho- ma. Results （1）The most cases are B cells in origin; （2）The eases of B cells in origin usually emerge at the foreside of orbit and lacrimal gland, the border is clear , not smooth, the texture is moderate and no pain; The cases of T cells in origin can be acute, just like orbital cellullitis, can be chronic just like orbital pseudotumor; （3）The manifestion of CT and MRI is not particular; （4）B small lymphoid cells is the generally type in pathology. Conclusion For the orbital non-Hodgkin＇s Lymphoma, we should do the pathological analysis as soon as fast for the best treatment opportunity.
